Abstract: The EXCITE ISR trial is the first large, prospective, randomized study to demonstrate superiority of ELA + PTA versus PTA alone for treating femoropopliteal ISR. (Randomized Study of Laser and Balloon Angioplasty Versus Balloon Angioplasty to Treat Peripheral In-stent Restenosis [EXCITE ISR]; NCT01330628).

“…Even the long-term benefit of a "stent sandwich" remains to be proven. The trial of excimer laser atherectomy with adjunctive POBA 21 (mean lesion length, 196 mm) showed superiority to standalone POBA alone with respect to 6-month freedom from TLR and 30-day major adverse events. The comparably higher rate of 6-month freedom from TLR after DCBA in FAIR may be attributable to shorter lesions.…”

“…There has been 1 study that now has gained Food and Drug Administration approval for therapy for in-stent restenosis. In the Excimer Laser randomized Controlled Study for Treatment of Femoropopliteal In-Stent restenosis (ExCITE) trial, 20 where 240 patients with stent restenosis were randomly assigned to laser atherectomy with angioplasty versus simple angioplasty alone, lesion lengths ranged over 30 cm in 20% patients with an average of 19 cm in each group. Total occlusions were between 30% and 36% in each group, respectively.…”

“…Excimer laser plus PTA was compared with PTA alone in 250 patients in Excimer laser Randomized Controlled Study for Treatment of Femoropopliteal In-Stent Restenosis (EXCITE ISR). 53 laser had higher procedural success (93.5% versus 82.7%, P=0.01) and lower, but still substantial, TlR (26.5% versus 48.2%, P<0.005) at a follow-up of only 6 months.…”

“…Numerous treatment options have been proposed to treat FP ISR. PTA alone is ineffective, with 6-month TlR of up to 48.2% 53 and 1-year patency rates of ≈28%. 54 Cutting balloon angioplasty is no better.…”
