“…Despite the study's highly selective eligibility criteria, radiation clearly reduced the risk of local-regional recurrences. These data are consistent with those from previously published prospective trials that also studied highly selected favourable patients and found increased rates of breast recurrences when radiation was omitted (Veronesi et al, 1993;Schnitt et al, 1996;Liljegren et al, 1997;Wollmark et al, 2000).…”

“…This risk approaches 30 -40% by 10 years after initial diagnosis and treatment. 52,53 Because the average patient-specific life expectancy (based on age, stage, and specific comorbidities) of black women in our study was 9 years (not shown), this pattern of care may place women at substantial risk for disease recurrences within their lifetimes. 54,55 Black women are also more likely to have a less favorable pattern of disease recurrence than whites, with associated decrements in quality of life.…”

“…Small tumour size, low grade and old age have been proposed as selection criteria for patients who might not benefit from radiotherapy (Clark et al, 1992;Veronesi et al, 1993;Liljegren et al, 1994). However, in a highly selected group of 87 patients treated with breast-conserving surgery without radiotherapy, the rate of local recurrence rose to 16% when the median follow-up time was 56 months, therefore this prospective clinical trial was closed .…”

“…There is a need to investigate optional or additional treatment modalities such as more effective systemic therapies for women with a risk of local recurrence after limited surgery and irradiation (Gelber and Goldhirsch, 1994). Attempts have also been made to find a subgroup of low-risk patients who do not need radiotherapy (Clark et al, 1992;Veronesi et al, 1993;Liljegren et al, 1994).…”
