Quantitative MRI Reveals Microstructural Changes in the Upper Leg Muscles After Running a Marathon

Abstract: Background The majority of sports‐related injuries involve skeletal muscle. Unlike acute trauma, which is often caused by a single traumatic event leading to acute symptoms, exercise‐induced microtrauma may remain subclinical and difficult to detect. Therefore, novel methods to detect and localize subclinical exercise‐induced muscle microtrauma are desirable. Purpose To assess acute and delayed microstructural changes in upper leg muscles with multiparametric quantitati… Show more

“…The MRI protocol included proton density‐weighted (PD), T 2 ‐weighted (T2w) and DWI in an axial slice order from proximal to distal (total acquisition time 27 min). To avoid shimming artifacts due to large field of view (FOV), the upper leg region was divided into three FOVs of 480 × 264 × 150 mm 3 along the z ‐axis (stacks) 25 . For accurate merging the stacks had an overlap of 10 mm.…”

“…7,8 Our group investigated changes in DTI indices in the upper leg muscles during recovery after a marathon. 9,10 Distinct changes in DTI indices were detected in muscles that appeared normal on T 2 -weighted images, demonstrating the added value of DTI as a sensitive readout of muscle integrity.…”

“…Furthermore, in sport medicine, DTI has shown promise as a relevant tool to understand the muscle recovery process that follows physical activity and after injury 7,8 . Our group investigated changes in DTI indices in the upper leg muscles during recovery after a marathon 9,10 . “…In skeletal muscle, the highest diffusivity is observed along the axis of the fiber [axial diffusivity (AD)] and the lowest in the fiber cross section [radial diffusivity (RD)]. Due to its exquisite sensitivity to tissue microstructure, DTI is becoming an increasing popular tool to assess skeletal muscle status in a wide range of diseases ( 1 ) and muscle injuries ( 2 ).…”
