Pyoderma gangrenosum and monoclonal gammopathy

Abstract: The records of eight patients with pyoderma gangrenosum and monoclonal gammopathy showed that all patients except one had an IgA paraproteinemia. To date, seven patients have had a benign course and multiple myeloma has developed in one. In seven patients, the onset of the pyoderma gangrenosum preceded the detection of the monoclonal gammopathy. The monoclonal gammopathy did not seem to influence the morphologic findings, course, or therapy of the pyoderma gangrenosum. In the one patient with myeloma, treatmen… Show more

“…The causal relationship between monoclonal gammopathy and PG is not established, and the incidence of the association varies among studies. Powell et al 12 report that 9 of 86 patients with PG had monoclonal gammopathy: 7 had IgA paraprotein, 1 had immunoglobulin G (IgG), and 1 had immunoglobulin M (IgM). Prystowsky et al 13 reviewed 21 patients with PG, 4 of whom had IgA monoclonal gammopathy.…”

“…There is a well-described association between monoclonal gammopathy and another dermatitis, pyoderma gangrenosum [11, 12, 13, 14]. This is a rare condition that has classically been linked to inflammatory gastrointestinal disorders such as ulcerative colitis; however, it has also been found to be associated with rheumatoid arthritis, monoclonal gammopathies and occasionally lymphomas and leukemias.…”

“…PG has been associated commonly with inflammatory bowel disease, arthritis, malignant disorders and monoclonal gammopathy, usually IgA [10, 11, 12, 13, 14, 15, 16, 17]. PG has rarely been reported in patients with SLE [12, 22, 23, 24, 25], SLE with secondary antiphospholipid antibody syndrome [20, 21]or patients with primary antiphospholipid antibody syndrome [19, 26].…”

“…PG has been associated most commonly with inflammatory bowel disease, arthritis and hematologic premalignant or malignant disorders [10, 11, 12, 13, 14, 15, 16, 17]. Although PG-like ulceration has been reported in patients with the antiphospholipid antibody syndrome, PG has rarely been reported in patients with SLE [18, 19, 20, 21, 22, 23, 24, 25, 26].…”
