Pulmonary Adenocarcinomas of the Fetal Lung Type

Abstract: Seven cases of high-grade adenocarcinoma of fetal lung type (H-FLAC) are compared with nine cases of pulmonary endodermal tumor resembling fetal lung or low-grade adenocarcinoma of fetal lung type (L-FLAC). Of the seven patients with of H-FLAC, four were men and three were women. All of the patients but one were in their 60s or 70s. Five patients were smokers. After resection of the tumor, three patients died of metastases, two patients are alive with no evidence of disease, and two patients died of a postoper… Show more

“…as a type of pulmonary adenocarcinoma of the fetal lung 10. Those authors divided pulmonary adenocarcinomas of the fetal lung into low‐ and high‐grade forms, and low‐grade adenocarcinomas of fetal lung type (L‐FLACs) are also known as well‐differentiated fetal adenocarcinomas (WDFAs) in the WHO classification at present 10. Currently, both L‐FLACs/WDFAs and H‐FLACs are included as a subtype of adenocarcinoma in the WHO classification 11.…”

“…When mixtures occur with other histologic subtypes, the tumor should be classified according to the predominant component. 158 This tumor typically occurs in younger patients than other adenocarcinomas. Uniquely, these tumors appear driven by mutations in the beta-catenin gene, and the epithelial cells express aberrant nuclear and cytoplasmic staining with this antibody by immunohistochemistry.…”

“…It appears as a fetal lung at 10 to 15 weeks of gestation, resembling pulmonary blastoma, but the adjacent mesenchymal tissue is histologically benign (10). Recent WHO 2002 classification removed WDFA tumours from the pulmonary blastoma category and classified them as a variant of adenocarcinoma (11).…”
