Amaç: İdiyopatik Parkinson hastalığında (İPH) oksidatif stres nöronal hücre ölümü üzerinde majör role sahiptir. Serüloplazmin ferrooksidaz aktivitesi ile ferröz demiri ferrik forma okside eder ve böylece hücrenin DNA, lipid ve protein bileşenlerini oksidatif hasardan korur. Bizim çalışmamızda proton manyetik rezonans spektroskopi (H-1 MRS) ile serüloplazmin eksikliğinin nörodejenerasyon üzerindeki etkisi araştırılmıştır. Gereç ve Yöntem: İPH tanılı 23 hasta ve 12 sağlıklı kontrol olguda iki yanlı putamen tek voksel H-1 MRS yöntemiyle incelendi. Her iki grupta N-asetil aspartat (NAA), kreatin (Cr) ve kolin (Cho) pik değerleri ve serum serüloplazmin düzeyleri belirlendi. Bulgular: İPH hastalarında semptomatik ekstremitenin kontralateral (KL) putaminal bölge NAA/Cho ve NAA/Cr oranı kontrol grubuna kıyasla anlamlı derecede düşüktü. Ayrıca İPH grubunda KL NAA/Cho ve NAA/Cr oranları ipsilateraline (İL) göre anlamlı derecede düşüktü (p<0,001). Semptomatik ekstremitenin İL putaminal bölgeye ait metabolit oranları karşılaştırıldığında sağlıklı kontrol ile İPH grubu arasında anlamlı fark saptanmadı. Serum serüloplazmin seviyeleri kontrol grubu ile karşılaştırıldığında İPH grubunda anlamlı derecede düşük saptandı (p<0,001). İPH grubunda KL putaminal NAA/Cho oranı ile serüloplazmin seviyeleri arasında anlamlı korelasyon saptandı (p=0,011). Sonuç: Çalışmamızda saptanan serum serüloplazmin düşüklüğü ile KL putamen NAA/Cho oranı arasındaki bu korelasyon serbest radikal aracılı nöronal hücre ölümünde serüloplazmin yetersizliğinin katkıda bulunabileceği hipotezini desteklemektedir. Anahtar Kelimeler: İdiyopatik Parkinson hastalığı, proton manyetik rezonans spektroskopi, serüloplazmin Objective: Oxidative stress has been implicated to play a major role in the neuronal cell death in idiopathic Parkinson's disease (IPD). Ceruloplasmin is a ferroxidase that oxidizes toxic ferrous iron to its nontoxic ferric form and thus helps prevent oxidative damage to proteins, lipids, and DNA. The aim of this study was to determine the serum ceruloplasmin levels in patients with IPD and evaluate its role in neurodegeneration using hydrogen proton magnetic resonance spectroscopic imaging (H-1 MRSI). Materials and Methods: Twenty-three patients with the IPD and 12 healthy controls were studied using single-voxel H-1 MRSI of the bilateral putamen. The peak ratios of N-acetyl aspartate (NAA) to creatinine (Cr) and choline (Cho) were measured in both groups and serum ceruloplasmin levels were detected. Results: Compared with the controls, the ratios of NAA/Cho and NAA/Cr in putamen contralateral (CL) to the symptomatic limbs were significantly lower in patients with IPD. Also in the IPD group, CL NAA/Cho and NAA/Cr ratios were significantly lower than the ipsilateral (IL) values (p<0.001). IntroductionThe accurate diagnosis and monitoring of idiopathic Parkinson disease (PD), a progressive neurodegenerative disorder, has not been fully developed. This study sought to identify a neurochemical profile in multiple regions of the PD brain and healthy controls by proton magnetic resonance spectroscopy (1H‐MRS). We aimed to track changes of the brain neurochemical, quantify neuronal loss, and further determine the diagnostic value of 1H‐MRS.Methods PD patients and healthy controls recruited from Second Affiliated Hospital of Shantou University Medical College, Shantou, southern China, underwent 1H‐MRS. Chemical information was obtained for ratios of N‐acetylaspartate to creatine (NAA/Cr), NAA to choline (NAA/Cho), and Cho to Cr for substantia nigra, globus pallidus, prefrontal lobe, hippocampus, cuneus gyrus, and dorsal thalamus regions.ResultsCompared to the 20 healthy controls (12 male, age 58.75 ± 5.03 years), the 42 patients (21 male, age 61.60 ± 6.40 years) showed lower NAA/Cr and NAA/Cho ratios in substantia nigra, globus pallidus, prefrontal lobe, hippocampus, cuneus gyrus and dorsal thalamus regions (p < .01); NAA/Cr and NAA/Cho ratios were reduced for both patients with unilateral and mild/no cognitive impairment (p < .01); Unified Parkinson's Disease Rating Scale score was inversely correlated with NAA/Cr ratios in the substantia nigra (r = −.32; p = .042).Conclusion NAA/Cr and NAA/Cho ratios may be useful metabolic biomarkers for early diagnosis of PD. Multi‐voxel 1H‐MRS can provide information on brain neurochemistry and may be a promising technique for diagnosis of and monitoring neuronal loss in PD.

-The differential diagnosis of Parkinsonism based on clinical features, sometimes may be difficult. Diagnostic tests in these cases might be useful, especially magnetic resonance imaging, a noninvasive exam, not as expensive as positron emission tomography, and provides a good basis for anatomical analysis. The magnetic resonance spectroscopy analyzes cerebral metabolism, yielding inconsistent results in parkinsonian disorders. We selected 40 individuals for magnetic resonance imaging and spectroscopy analysis, 12 with Parkinson's disease, 11 with progressive supranuclear palsy, 7 with multiple system atrophy (parkinsonian type), and 10 individuals without any psychiatric or neurological disorders (controls). Clinical scales included Hoenh and Yahr, unified Parkinson's disease rating scale and mini mental status examination. The results showed that patients with Parkinson's disease and controls presented the same aspects on neuroimaging, with few or absence of abnormalities, and supranuclear progressive palsy and multiple system atrophy showed abnormalities, some of which statistically significant. Thus, magnetic resonance imaging and spectroscopy could be useful as a tool in differential diagnosis of Parkinsonism.KeY WorDs: Parkinson's disease, progressive supranuclear palsy, multiple system atrophy, magnetic resonance, spectroscopy. A ressonância por espectroscopia analisa o metabolismo cerebral, com resultados variáveis na literatura no estudo das síndromes parkinsonianas. selecionamos 40 indivíduos para realização de ressonância magnética e espectroscopia, sendo 12 com doença de Parkinson, 11 com paralisia supranuclear progressiva, 7 com atrofia de múltiplos sistemas tipo parkinsoniana e 10 indivíduos sem manifestações neurológicas ou psiquiátricas (grupo controle). As escalas clínicas analisadas foram a de Hoenh e Yahr, unified Parkinson's disease rating scale e o mini-exame do estado mental. os resultados encontrados revelaram que pacientes com doença de Parkinson e controle apresentavam em geral o mesmo aspecto por imagem enquanto os grupos paralisia supranuclear progressiva e atrofia de múltiplos sistemas com anormalidades, havendo significância estatística em algumas variáveis.
