Protein-losing Enteropathy after Fontan Operation for Tricuspid Atresia (Imperforate Tricuspid Valve)

Abstract: Protein-losing enteropathy occurred in a 7-year-old girl with tricuspid atresia, concordant ventriculo-arterial connexions and a relatively large hypoplastic right ventricle, one year after an atrio-ventricular type of Fontan operation by means of a valveless woven Dacron conduit. Severe conduit regurgitation and a marked enlargement of the hypoplastic right ventricle were demonstrated at recatheterization. Insertion of a bioprosthetic valve at the base of the right atrial appendage led to a dramatic clinical … Show more

“…[1][2][3][4][5] The median time interval between surgery and diagnosis is 2.7 years (range: 0.1-16.4 years), but up to one third of cases develop at least 5 years after surgery. 4 Although PLE post-Fontan operation is generally thought to result from raised systemic venous pressure leading to increased lymph production and decreased lymph drainage resulting in lymphangiectasia, 23% of cases of PLE have favorable hemodynamics, 4 as in the current case, and cardiac surgery is beneficial in only 50% of survivors.…”

“…[148][149][150][151] A high level of clinical suspicion for PLE in patients with Fontan physiology is important because many patients remain asymptomatic during early stages of the disease. Others may present with chronic diarrhea or with secondary manifestations of hypoalbuminemia such as peripheral edema, ascites, clotting abnormalities, or recurrent infections.…”

“…W wyniku długotrwale utrzymującego się niskiego rzutu serca dochodzi do uruchomienia mechanizmów kompensacyjnych i redystrybucji przepływu krwi do ważnych dla życia narzą-dów, kosztem innych, na przykład przewodu pokarmowego. U pacjentów po operacji Fontana, szczególnie u tych, u których stwierdzono PLE, wykazano podwyższony opór naczyń krezkowych [10][11][12].…”
