Proposal for best practice in the use of video-EEG when psychogenic non-epileptic seizures are a possible diagnosis

Abstract: HighlightsThere is little standardization of EEG practices, when PNES is suspected.Use of suggestion is controversial but potential concerns can be addressed.Ictal assessment during PNES is essential and can strengthen the diagnosis.Optimal formatting of the EEG report can improve communication between clinicians.

“…For psychogenic TLOC, pharmacological provocation may be used as for reflex syncope for two reasons: firstly, VVS occurs more often in psychogenic pseudosyncope than chance predicts (Blad et al, 2015) and the occurrence of psychogenic TLOC may depend on suggestion (Whitehead et al, 2017), which may be aided by a provocation procedure. The same protocol may be used for psychogenic TLOC as for reflex syncope, except for the endpoint.…”

“…Routine EEG may be helpful to facilitate a diagnosis of psychogenic nonepileptic seizures (PNES) [80] . One study of 74 patients suspected of having PNES on clinical grounds analyzed the yield of short-term, outpatient EEG with video monitoring (OVEM).…”
