Critically ill children admitted to the pediatric intensive care unit (PICU) from 75 to 166 per 100,000 children/year including respiratory diseases, infectious diseases, congenital diseases, and perioperative care. Early detection of disease severity in critically ill children uses the PELOD-2 score. Previous studies have shown that neutrophil lymphocyte ratio (NLR) can be used as a predictor of pediatric mortality and monocyte and lymphocyte ratio (MLR) to determine the severity of Klebsiella pneumonia infection. The aim of this study is to determine the correlation of NLR and MLR to PELOD-2 score in critically ill children in the PICU of Haji Adam Malik General Hospital. The research method is cross sectional analytic research from medical records, then documentation of PELOD-2 scores. The subjects were critically ill children from October to December 2022 according to the inclusion and exclusion criteria. Data were analyzed with the Spearman correlation test. Results: A total of 59 pediatric patients, the most was female 34 patients (57.6%). The mean age of the children was 6.23 years old. With the most indications was respiratory disease 22 people (37.3%). Spearman correlation test analysis showed that there was no significant correlation between NLR and MLR with PELOD 2 on the first day (p=0.316 and p=0.696) and MLR with PELOD 2 on the third day of treatment (p=0.077). However, a significant correlation was found between NLR and PELOD 2 in critically ill children on the third day of care (p=0.014). Conclusion: There was a significant correlation between NLR and PELOD-2 score on the third day of care. However, there was no correlation between first and third day MLR and first day NLR with PELOD-2 score.

Pendahuluan: Pasien anak yang dirawat di Pediatric Intensive Care Unit (PICU) merupakan populasi rentan dengan derajat morbiditas dan mortalitas yang tinggi. Pemeriksaan laboratorium darah (darah rutin dan biomarker infeksi) merupakan pemeriksaan penunjang yang sering digunakan untuk mengevaluasi kondisi pasien anak. Selain itu beberapa metode skoring seperti skor Pediatric Logistic Organ Dysfunction (PELOD-2) juga sering dipakai untuk menilai derajat keparahan pasien anak. Pemeriksaan untuk menilai skor PELOD-2 tidak selalu dapat dilakukan di fasilitas kesehatan sekunder dengan sumber daya terbatas. Pemeriksaan angka-leukosit, rasio neutrofil-limfosit, CRP dan Prokalsitonin diharapkan dapat menjadi alternatif pemeriksaan ketika skoring PELOD-2 tidak dapat dilakukan di fasilitas kesehatan. Penelitian ini bertujuan untuk melihat hubungan antara angka leukosit, rasio neutrofil-limfosit, CRP dan Prokalsitonin dengan skor PELOD-2. Metode: Penelitian ini menggunakan studi retrospektif komparatif pada 75 pasien anak yang dirawat di PICU RSCM sejak bulan Januari 2019-Agustus 2020 dan telah lulus uji etik. Pasien anak yang memiliki data tidak lengkap merupakan kriteria eksklusi penelitian. Untuk melihat hubungan variabel secara independen terhadap skor PELOD-2 digunakan analisis korelasi Spearman. Regresi logistik digunakan untuk mengevaluasi hubungan masing-masing variabel independen secara bersamaan dengan skor PELOD-2. Hasil: Pada analisis korelasi Spearman, didapatkan hubungan yang signifikan antara nilai leukosit (0,059; IK90%) dan Prokalsitonin (0,076; IK90%) dengan skor PELOD-2. Dari hasil regresi logistik juga ditemukan bahwa parameter yang signifikan dalam mempengaruhi skor PELOD-2 adalah angka leukosit dan Prokalsitonin. Kesimpulan: Nilai leukosit dan kadar Prokalsitonin bisa digunakan sebagai alternatif untuk menilai derajat keparahan anak ketika skor-PELOD2 tidak memungkinkan untuk dikerjakan di fasilitas kesehatan.

Latar belakang. Sepsis merupakan penyebab utama morbiditas dan mortalitas utama pada anak di seluruh dunia. C-reactive protein (CRP) merupakan penanda infeksi yang paling banyak digunakan, tetapi parameter tersebut terkendala tingkat sensitivitas, spesifisitas, ketersediaan alat, dan biaya. Neutrophil-lymphocyte Count Ratio (NLCR) merupakan pemeriksaan yang mudah dan murah, serta banyak digunakan sebagai penanda diagnostik pada berbagai proses inflamasi.Tujuan. Menganalisis korelasi antara NLCR dan CRP pada pasien sepsis anak.Metode. Studi analitik observasional menggunakan data sekunder dari register sepsis anak di RSUP Dr. Hasan Sadikin Bandung periode Januari 2019–Desember 2019. Pengambilan data berupa karakteristik pasien, nilai NLCR, dan CRP, didapatkan data tidak berdistribusi normal( uji Kolmogorov-Smirnov) maka analisis korelasi dilakukan menggunakan uji Rank Spearman dengan nilai kemaknaan p<0,05.Hasil. Sebanyak 80 subjek memenuhi kriteria inklusi. Median nilai NLCR subjek 2,98 (rentang 0,12-19,38) dan nilai median CRP subjek 2,44 (rentang 0,01-34,04) Nilai NLCR memiliki korelasi bermakna dengan CRP (r=0,310; p=0,005). Kesimpulan. Terdapat korelasi positif yang bermakna antara neutrophil-lymphocyte count ratio dengan CRP pada pasien sepsis anak.
