Prognostic Significance of PCNA Expression in Laryngeal Cancer
Abstract: To assess the prognostic value of proliferating cell nuclear antigen (PCNA) in laryngeal carcinoma and its relation with other known prognostic clinicopathologic variables. Design: A retrospective cohort study of 92 patients chosen randomly from patients treated between 1964 and 1993 with the diagnosis of laryngeal cancer. Prognostic factors including PCNA expression, grade, lymphovascular invasion, depth of tumor margins, neck metastasis, and clinical outcome were evaluated.

“…Aggressive, poorly differentiated tumors that are prone to metastasize are rich in DNA alterations and could thus express PCNA in many cells that are not engaged in the cell cycle and therefore not expressing Ki67. In our series, the percentage of PCNA‐ or Ki67‐immunolabeled cancer cells was inversely correlated with the degree of differentiation, in agreement with previous studies 15–20…”

“…[15] reported similar results, demonstrating higher PCNA index in metastases than in primary carcinomas. Additionally, we have shown that laryngeal tumours associated with neck metastases were characterized by increased PCNA index when compared with tumours without lymph node metastases, which is consistent with a great number of studies [11–17] and in contradiction to only a few reports [20–22]. It is plausible that metastasizing laryngeal carcinomas could develop a cell population with highly proliferative potential, and this potential is retained in arising metastases.…”

“…Correlation of PCNA status with other clinicopathological parameters showed only signi®cance with lymph node status. A similar correlation was also reported by Sarac et al [26] probably indicating that PCNA index is a more sensitive variable than grade in predicting lymph node metastasis and prognosis. Direct correlation was evident between presence of HPV and expression of PCNA, indicating increased proliferation in tumors positive for HPV.…”
