Background: We assessed whether immunohistologic markers for glomerular or tubulointerstitial injury might provide better correlations with ongoing renal function and disease activity as compared with the WHO classification or the NIH activity and chronicity indices in lupus nephritis. Methods: Thirty-three patients with clinically defined systemic lupus underwent renal biopsy over a 1-year period at Hospital Loayza in Lima, Peru. Biopsy specimens were evaluated for macrophages, proliferating cells, α-actin expression, and type IV collagen deposition in both glomeruli and the tubulointerstitium and the results compared with the current WHO and NIH classifications in relation to the clinical presentation. Results: Patients with WHO class IV lupus nephritis were more likely to have lower serum complements, greater proteinuria and hematuria, and worse renal function. An elevated NIH activity index correlated with microhematuria, proteinuria, and impaired renal function, whereas an elevated chronicity index correlated with renal function, hypertension, and microhematuria, but not with proteinuria. The presence of glomerular macrophages correlated with both glomerular α-actin expression and type IV collagen deposition, but did not correlate with renal function or proteinuria. In contrast, interstitial macrophages correlated not only with interstitial collagen deposition and myofibroblast accumulation, but also correlated with both renal function and the presence of nephrotic syndrome. Conclusions: Both the WHO classification and the NIH activity/chronicity indices correlate with clinical manifestations of lupus nephritis. While glomerular macrophage accumulation correlates with mesangial cell activation (α-actin expression) and collagen deposition, and interstitial macrophage accumulation correlates with interstitial fibroblast activation and collagen deposition, only interstitial macrophages correlate with renal function. Of particular interest will be future studies to determine whether these markers correlate with the prognosis.

Over 10 years have passed since the latest revision of the histopathologic classification of lupus nephritis. This revision was a significant improvement compared with the previous version, mainly because of clearer and more concise definitions and the elimination of mixed subclasses. Despite these improvements, there are still some difficulties in the classification for lupus nephritis, many of which are in the definitions provided. In this review, we focus on the difficulties surrounding the evaluation of classes III and IV lesions, particularly the definitions of endocapillary and extracapillary proliferation, the use of the terms endocapillary proliferation and hypercellularity, the clinical relevance of segmental and global subdivision in class IV, and the value of distinguishing lesions that indicate activity and chronicity. Vascular and tubulointerstitial lesions are also discussed. Furthermore, we give an overview of the history of the classification to provide background on the origin and development of the definitions in lupus nephritis. The issues raised in this review as well as the suggestions for improvements may assist with a revision of the lupus nephritis classification in the near future.

RESUMO -OBJETIVO. Estudar a apresentação clí INTRODUÇÃOA sobrevida de pacientes com lupus eritematoso sistêmico (LES) vem aumentando no correr dos anos. A melhora pode ser atribuída à vários fatores, tais como diagnóstico de casos mais leves, melhor controle da pressão arterial, introdução de novos antibióticos, uso de corticosteróides e de drogas imunossupressoras [1][2][3][4] . Vários fatores tais como raça negra 1,5-9 , desenvolvimento de glomerulonefrite [8][9][10][11][12][13][14] , baixo nível só-cio-econômico 1,9 têm sido apontados como indicadores de pior prognóstico.Nos países desenvolvidos, a importância de septicemia como causa de óbito vem diminuindo. Estudos recentes têm relatado que a doença cardiovascular está se tornando importante causa de óbito 14,15 . Em países em desenvolvimento é possível que os pacientes com glomerulonefrite lúpica apresentem pior prognóstico devido a associação de infecção, baixos níveis sócio-econômicos e baixa escolaridade, usualmente encontrados nesses países. O objetivo do presente trabalho foi avaliar a apresentação clínica e evolução da glomerulonefrite lúpica em nosso meio. variáveis estudadas, à época da biopsia renal, (idade, sexo, proteinúria, presença de hipertensão arterial e creatinina sérica) a única que se associou com pior prognóstico foi a elevação da creatinina sérica. Remissão da síndrome nefró-tica ocorreu em 65% das vezes. A sobrevida atuarial foi de 96%, 82%, 70% e 70% em 1, 5, 10 e 12 anos. Cinco pacientes desenvolveram insuficiência renal crônica terminal e sete morreram, sendo infecção a principal causa de óbito (57%) CONCLUSÃO. Em pacientes com nefropatia lú-pica, o aumento da creatinina sérica, à época da biópsia, se associou com o desenvolvimento de insuficiência renal crônica ao fim do seguimento e a principal causa de óbito foi processo infeccioso.Unitermos: Lupus eritematoso sistêmico. Insuficiência renal.Glomerulonefrite lúpica: estudo da evolução a longo prazo CASUÍSTICA E MÉTODOSForam analisados retrospectivamente os prontuários de todos os pacientes com o diagnóstico de lupus eritematoso sistêmico (LES) atendidos na disciplina de Nefrologia da Faculdade de Medicina de Botucatu, UNESP, no período de 1970 a 1993. Para ser incluído no trabalho, o paciente deveria ter idade superior a 12 anos no início do seguimento, preencher pelo menos quatro dos critérios da American Rheumatism Association 15 para diagnóstico de LES, ter sido submetido à biópsia renal, apresentar seguimento mínimo de um mês e não necessitar de tratamento dialítico quando visto pela primeira vez.Considerou-se o paciente como portador de insuficiência renal, quando este apresentava creatinina sérica superior a 1,5mg/dl. Aceitou-se como piora da função renal aumento superior a 30% da creatinina sérica inicial em mais de uma ocasião e melhora quando ocorreu diminuição semelhante.Definiu-se como síndrome nefrótica a presença de proteinúria superior a 3 g/24h; como remissão completa da síndrome nefrótica quando a proteinúria de 24 hora...
