Background: Diabetic infections are frequent and the etiologies are multiple. The present study aims to identify the etiologies of the infections of the diabetic patient hospitalized in the University Teaching Hospital of Bouake in Côte d’Ivoire. Materials and Methods: This was a retrospective cross-sectional study conducted in the Internal Medicine Department from January 2019 to December 2020. The study population consisted of hospitalized and infected diabetic patients. Of this study population we included in the study 136 patients. Data analysis was done with Epi Info 7.2.3.1 software. Results: The prevalence of infection in hospitalized diabetics was 75.1%. The mean age of the patients was 52 ± 13.4 years. The sex ratio was 0.7. Diabetes was incidentally discovered in 50% and type 2 diabetes (88.2%) predominated. The reasons for hospitalization were dominated by ketoacidosis (58.1%), glycemic imbalance (19.1%) and hyperosmolar hyperglycemia syndrome (10.3%). Fever was present in 41.2% of cases. The infectious foci were urinary tract infections (29.4%), pneumopathies (28.7%), malaria (21.3%), skin infections (13.2%) and the undetermined focus (7.3%). The infectious focus was unique in 90.4%. The germs identified were plasmodium (21.3%), Escherichia coli(8.8%), staphylococcus (8.3%), yeasts (8.3%) and Enterobacter (6.7%). Beta-lactams (75.6%) were the most prescribed anti-infective treatment. Mortality was 14.7% related to type 1 diabetes (p=0.001), duration of diabetes greater than 5 years (p=0.005), hospitalization latency greater than 7 days (p=0.001), mucocutaneous focus (p=0.005) and Undetermined foci (p=0.001). Conclusion: Diabetic infections are frequent and the etiologies are varied. They must be systematically sought in hospitalized diabetics.

Background: In order to promote the Ivorian pharmacopoeia, we undertook the study of Combretum micranthum (Combretaceae), a plant commonly used in Ivory Coast for the treatment of diabetes. The aim of this study was to evaluate the hypoglycemic effects of an aqueous leaves extract of Combretum micranthum (EaqCM) in Wistar rats. Methods and Materials: A phytochemical screening study was performed. The pharmacological test on normoglycaemic rats was carried out in 4 groups of 5 rats per group. Rats in group 1 received 2 ml of distilled water, and rats in the test groups received doses ranging at 100 to 300 mg/kg B.W. of the EAqCM. Tests in pre-treated temporarily hyperglycemic rats were performed in 4 groups of 5 rats per group. The rats in groups 1 and 2 received 2ml of distilled water, the rats in test groups (3 and 4) received the dose of 10-2g/kg B.W. and 100 mg/kg B.W. respectively glibenclamide and EAqCM. Thirty minutes later, all rats except those in group 1 received anhydrous glucose. The protocol for testing post-treated hyperglycaemic rats is the same as that for pretreated rats, except that in this experiment glucose is administered to the animals before the test substances. Blood glucose is measured in the rats at regular intervals of 30, 60, 90, 120, 150, and 180 minutes after administration of the substances. Results: Qualitative phytochemical analysis of EAqCM revealed the presence of polyphenols, flavonoids, catechic, tannins, quinonic compounds, alkaloids, sterols, and polyterpenes. Pharmacological tests on the glycemia of normoglycemic rats showed that EAqCM has hypoglycemic properties at a dose of 100 mg/kgB.W. with a percentage reduction of 30%. In hyperglycemic rats, EAqCM and glibenclamide exhibited anti-hyperglycemic activity with respective percentages reduction of 57.47 % and 58.82 %. Conclusion: Antihyperglycemic effect of EAqCM in post-treated rats would probably be due to the presence of polyphenols and flavonoids in this extract. These results support the use of this plant in traditional medicine for the treatment of diabetes.

Objectif : Identifier les étiologies des infections et déterminer les facteurs de risque de mortalité chez le diabétique à Bouaké. Patients et méthode : Il s’agissait d’une étude rétrospective menée en médecine interne de janvier 2020 à décembre 2021. La population d’étude était constituée de patients diabétiques hospitalisés et infectés. L’analyse des données a été faite avec le logiciel Epi Info 7.2.3.1. Résultats : La prévalence de l’infection chez le diabétique hospitalisé était de 75,1%. L’âge moyen des patients était de 52 ans. Le sex-ratio était de 0,7. Le diabète était de découverte fortuite dans 50% et le diabète de type 2 (88,2%) prédominait. Les motifs d’hospitalisation étaient dominés par l’acidocétose (58,1 %), le déséquilibre glycémique (19,1 %) et l’hyperglycémie (10,3 %). La fièvre était présente dans 41,2 %. Les principales infections étaient les infections urinaires (29,4%), les pneumopathies (28,7%), le paludisme (21,3%), les infections cutanées (13,2%) et le foyer indéterminé (7,3%). Les germes identifiés étaient le plasmodium (21,3%), Escherichia coli (8,8%), staphylocoque (8,3%), des levures (8,3%) et enterobacter (6,7%). Les bêtalactamines (75,6%) étaient le traitement anti infectieux le plus prescrit. La mortalité était de 14,7% en lien avec le diabète de type 1 (p=0,001), l’ancienneté du diabète supérieur à 5 ans (p=0,005), la latence d’hospitalisation supérieure à 7 jours (p=0,001), le foyer cutanéomuqueux (p=0,005) et le sepsis (p=0,001). Conclusion : Les infections du diabétique sont fréquentes et les étiologies sont variées. Elles doivent être systématiquement recherchées chez le diabétique hospitalisé
