Process mapping as a framework for performance improvement in emergency general surgery

Abstract: Background: Emergency general surgery conditions are often thought of as being too acute for the development of standardized approaches to quality improvement. However, process mapping, a concept that has been applied extensively in manufacturing quality improvement, is now being used in health care. The objective of this study was to create process maps for small bowel obstruction in an effort to identify potential areas for quality improvement. Methods:We used the American College of Surgeons Emergency Gener… Show more

“…In North America and the UK, process mapping has been used to identify sources of variation in management and areas of inefficiency in surgical care [26][27][28]. DiGirolamo et al [29] studied the management of small bowel obstruction and used process mapping to identify patient flow in the emergency department and operating room capacity as key sources of inefficiency. Likewise, McLaughlin et al [30] used process mapping in their institutional redesign of neurosurgical care to identify opportunities for care standardization.…”

“…Phase 2: 85% of the studies stated that data and information had been gathered to inform the PM exercise as a substitute for (55%), or in addition to (30%), the group knowledge generated in the facilitated process mapping sessions. This included evidence-based best practice recommendations [ 34 155 ], interviews [ 64 , 73 148 , 154 , 156 , 158 , 160 ], and more detailed approaches such as observations, operational data collection, time-and-motion studies, and video footage [ 48 , 78 81 , 141 , 143 146 , 150 , 159 , 163 169 ].…”

“…Process mapping has been used in emergency general surgery as well as cardiac surgery to isolate each segment of a patient’s experience, measure the times and look for significant variability between sequence and timing for different patients at each stage of their surgical journey. 19 , 20 An emergency general surgery model of care is an organizational structure that provides protected operating time for surgical emergencies, as well as structures and processes designed to improve the care of patients with general surgical emergencies. 21 Evidence from hospitals using the emergency general surgery model shows important benefits, including decreases in time to surgical evaluation, time to the operating room, length of stay, morbidity, mortality, cost and nighttime operating.…”
