Epidemiological evidence for the link between long-term exposure to air particulate matter (PM 2.5) and occurrence of type 2 diabetes (T2D) is limited such that little is known about the effect of PM 2.5 exposure and adult T2D prevalence. Thus, the aim of this ecological study is to evaluate the contribution of ambient PM 2.5 exposure to the adult T2D prevalence in the large population of Iran. The study was conducted based on Iran's large-scale cross-sectional surveilling non-communicable diseases (NCDs) risk factors (Timpka et al. 2015b). A total of 31,050 participants were enrolled in three sequential processes of study using cluster random sampling. PM 2.5 data in the urban area of 31 provinces of Iran were acquired from Tehran Air Quality Control Company (AQCC) and the Department of Environment (DoE) of Iran during 2012-2016. Moreover, major and minor diabetes risk factors were considered; the Pearson correlation and a stepwise regression model were performed to estimate associations between risk factors and diabetes and prediabetes prevalence. The results showed T2D prevalence was more frequent among women (10.61%) than men (9.35%). A weak positive correlation was observed between PM 2.5 level and diabetes prevalence with a correlation coefficient of 0.275, although there was no significant association between PM 2.5 value and prediabetes prevalence. Moreover, none of the variables included in the regression model could predict the prevalence of diabetes and prediabetes. According to our study results, it can be suggested that investigating the association between PM 2.5 exposure and T2D prevalence at individual level may provide a better understanding of PM 2.5 exposure and the risk of T2D prevalence.

Background: The present study aimed to assess the prevalence and intensity of catastrophic health-care expenditures (CHE) relating to type 2 diabetes mellitus care and inequality in facing such expenditures in Iran. Methods: A total of 1065 type 2 diabetes patients were included in this cross-sectional study. A multistage sampling method was used to select the samples. Data on sociodemographic characteristics, economic status, health and diabetic costs were collected using a selfconstructed questionnaire. We used capacity to pay (CTP) of households to calculate the incidence of CHE due to diabetic care at four different thresholds. The mean positive overshoot (MPO) and overshoot were used to assess the intensity of CHE. The relative concentration index and slope index of inequality (SII) were used to measure socioeconomicrelated inequalities in incidences of CHE. In addition, decomposition methods were used to identify the main factors affecting observed inequality in CHE. Results: The incidence of CHE at the 10, 20, 30, and 40% of CTP thresholds for type 2 diabetes mellitus care was 57.5, 28.9, 16.5, and 11.4%, respectively. The results of CI and SII indices for CHE due to diabetic care indicated that the incidence of CHE was more prevalent among patients with lower socioeconomic groups. The decomposition analysis showed that the socioeconomic status, marital status and gender of patients were the main factors contributing to socioeconomic inequality in incidence of CHE among the poor. Conclusion: Our study demonstrated that the incidence and intensity of CHE due to diabetic care were relatively high, particularly among socioeconomically disadvantaged patients. Modification to the present health care financing strategies is recommended in order to protect lower socioeconomic groups against the financial burden of diabetic care.

This study aimed to determine the prevalence of and demographic factors associated with domestic violence among Iranian older adults based on the Urban HEART-2 project in Tehran, Iran. The study was a part of the second round of the Urban Health Equity Assessment and Response Tool (Urban HEART-2) project, which was a large, populationbased, cross-sectional study. Individuals aged ≥ 60 years were selected randomly through a multistage, cluster sampling method from 368 neighborhoods of 22 districts of Tehran. A total of 15,069 older adults, 79% married and 45.22% female, were included in the study. The mean age of the participants was 68.93 ± 7.27 years. The overall prevalence of physical domestic violence was 2.32%. Factors such as age, gender, schooling, and having a disability were significantly associated with domestic violence (p < .05). These results are likely to be an underestimate of what really happens in the community, as many authors have mentioned previously. Therefore, it is necessary to broaden our perspectives on the phenomenon of domestic violence against the elderly, especially in the fields of public health and nursing.
