Breast cancer survivors (BCS) have identified attentional fatigue as a frequent and troublesome symptom during and following treatment. -4The ability to pay attention is necessary to complete tasks and activities, and problem solve. Attention requires ongoing mental effort. For BCS, the prolonged or intense use of attention to address the competing mental demands associated with illness, treatment and daily life activties, can lead to attentional fatigue and result in decreased effectiveness in activity. 5Attentional fatigue, a domain of cognitive function, is often used interchangeably with the terms cognitive dysfunction,cognitive impairment, cognitive fatigue or tiredness. Seminal research, however, delineates attentional fatigue from these other concepts by describing attentional fatigue as a measurable decrease in an individual's ability to focus and concentrate or to block out distractions when carrying out meaningful activities. 6 Hallmark indicators of attentional fatigue are: increased distractibility, difficulty following a train of thought or carrying out usual day-to-day activities, lapses in working memory, and increased impatience or frustration in personal interactions. 7Although the two may coexist, physical and attentional fatigue differ in that an individual may not be physically tired but has a reduced ability to exert mental effort. 8 Researchers have noted that BCS incur attentional fatigue more than age-matched controls 9 and prostate cancer patients. 10Merriman and colleagues, 11 found that up to 63% of

The problem: Some cancer patients have expressed attention, concentration and memory problems, during and after their treatment with chemotherapy, also called chemobrain. Following these complains, in recent decades, studies about this adverse effect had increased.Development of the topic: this work aims to review and analyze the latest scientific studies investigating the nature and severity of cognitive impairment associated with chemotherapy treatments. Works discussed are drowned from different bibliographical sources, mainly from the PubMed database. Results show that, in recent years, longitudinal studies have increased, chemobrain is being studied above all in samples from women with breast cancer, research in other types of cancer is insufficient. Also, the works include new assessment tools, such as MRI and EEG. Advances are done with animal experiments, in vivo and in vitro. Finally, we found studies on drugs and effective strategies for coping of cognitive impairment. The development of guidelines for its systematic study and recommendations for its approach are still pending.Conclusion: There is not conclusive data on the chemobrain, longitudinal studies should be encouraged, as well as assessing the cognitive impact of certain chemotherapeutic agents in different cancers. Como consecuencia de estas manifestaciones, en las últimas décadas los estudios dedicados a indagar este posible efecto adverso se han incrementado.Desarrollo del tema: Este trabajo pretende revisar y analizar los estudios científicos más recientes que investigan la naturaleza y alcance del daño cognitivo asociado a los tratamientos quimioterapéuticos. Los artículos analizados se han extraído de diferentes fuentes bibliográficas, principalmente de la base de datos PubMed. Como resultado de la búsqueda se aprecia que, en los últimos años, se han incremento los estudios de diseño longitudinal; este posible efecto adverso se sigue estudiando sobre todo en muestras de mujeres con cáncer de mama, las investigaciones en otros tipos de cánceres son insuficientes; así mismo, destaca la inclusión de nuevas herramientas de evaluación, como la resonancia magnética y el electroencefalograma, junto con nuevos experimentos realizados con animales, in vivo e in vitro, finalmente se constata la inclusión de algunos estudios sobre fármacos y estrategias eficaces para su afrontamiento. Chemotherapy-related cognitive impairment (CRCI) is commonly reported following the administration of chemotherapy to cancer patients, with most confirmatory results obtained from the cohort of breast cancer patients. The aims of the present study were to assess whether CRCI is consistently observed in people treated for colorectal cancer, to assess the impact of different treatment regimens, and to identify the domains of cognition affected.This study comprised four sample groups, three of which had been diagnosed and treated for colorectal cancer; chemotherapy patients (n = 19), patients treated with chemotherapy and the anti-angiogenic drug Avastin TM (n = 12), and surgery only patients (n = 10). A fourth, comparably aged and educated healthy control group was also included (n = 20). Each participant undertook approximately 90 minutes of testing, comprising nine neuropsychological tests, including a measure of everyday problem solving and self-report measures of anxiety, fatigue, depression and cognition.Multivariate analysis of variance revealed no significant differences between the groups across the neuropsychological test total scores. However, a significant difference was found when those tests comprised of subscales were broken down into their components; comparison between the surgery and healthy control groups found a difference on the delayed recall component of the logical memory test, with the surgery group having performed more poorly. Significant relationships were found between years of education, premorbid ability and everyday problem solving ability and cognitive functioning. The current study identified no significant increase in cognitive impairment related to chemotherapy and anti-angiogenic drugs used to treat colorectal cancer.
