“…Indeed, recipients receiving dialysis before transplantation were found to classically have a poorer 1-year survival rate compared with those undergoing preemptive SPKT (17). In another study, Pruijm et al (18) failed to demonstrate any benefit of being dialyzed before transplantation, on the pancreas, kidney, or in patient long-term survival. Paradoxically, dialysis was shown to improve platelet functional abnormalities related to uraemia and to reduce the risk of hemorrhage, the main cause of relaparotomy identified in our study (19,20).…”

“…Recipients of a preemptive SPK have better survival 8 years after transplantation when compared with those transplanted while already on dialysis [7]. Registry data also suggest that preemptive SPK is associated with improved kidney allograft survival compared to non-preemptive SPK [8].…”

“…Additional advantages of preemptive transplant over other treatments in the young are improved growth and quality of life [14]. Patients with diabetes also gain a survival advantage with PKT [9,15]. Other benefits include cost savings through dialysis avoidance, lack of the need for a Possible limitations of the data supporting PKT include the impact of lead time bias for PKT recipients as well as the unattainable goal of considering all recipient characteristics, surgical factors and practice patterns that may impact outcome [13].…”
