Abstract: Terlipressin plus albumin is an effective treatment for type 1 hepatorenal syndrome (HRS), but approximately only half of the patients respond to this therapy. The aim of this study was to assess predictive factors of response to treatment with terlipressin and albumin in patients with type 1 HRS. Thirty-nine patients with cirrhosis and type 1 HRS were treated prospectively with terlipressin and albumin. Demographic, clinical, and laboratory variables obtained before the initiation of treatment as well as chan… Show more

“…Our observations regarding factors predictive of HRS reversal and survival essentially confirm what has been observed in previous studies,22, 23, 24, 25, 26, 27 with the exception of the absence of a precipitating cause for HRS‐1, absence of prior rifaximin use in the terlipressin group, and male sex in the placebo group – all new observations. The absence of prior rifaximin use as a factor predictive of HRS reversal in the terlipressin group is of interest; it is possible that patients without prior rifaximin exposure are less ill than those receiving rifaximin.…”

“…Terlipressin günlük pratikte en fazla klinik çalışma yapılan vazopressin analoğudur. Özellikle albümin ile tedavide kombine edildiğinde tip 1 HRS'de renal fonksiyonları anlamlı şekilde düzelttiği (yaklaşık %34-75) görülmüştür (26,27). Ornipressin ve vazopressin'in dopamin ile beraber kullanıldığında böbrek kan akımını iyileştirdiği gözlenmiştir (28).…”

“…The mechanisms leading to decreasing cardiac output remain largely unknown, but SIRS has been suggested to be involved (see systemic haemodynamics and cardiac dysfunction). Gold standard treatments for HRS, which are directed to the restoration of arterial effective blood volume, are unsuccessful in 54% of patients, 79 suggesting that in these patients different factors contribute to the development of renal dysfunction. Moreover, in 30e40% of cirrhotic patients with renal failure bacterial infection, e.g.…”
