Predictor factors for choledocholithiasis

Abstract: Background The choledocolithiasis has an incidence of 8-20% in patients with cholecystolithiasis. The preoperative diagnosis guides the interventional treatment on the bile duct Aim To evaluate the sensitivity and specificity of the laboratory markers and imaging studies for choledocholithiasis preoperatively. Methods The study comprised 254 patients divided into two groups: the control group (207 patients), patients without choledocholithiasis intraoperatively and cases group (47 patients), that enrolled the p… Show more

“…EUS has an established role in the diagnosis of choledocholithiasis (CDL) and biliary tract disease. [123] The accuracy of EUS is superior to endoscopic retrograde cholangiography, without the associated risks, especially pancreatitis. [4567] A dilated common bile duct (CBD) is defined as having a diameter of >7 mm and is common in older people and those with previous cholecystectomy or chronic narcotic use.…”

“…[6789] Patients with dilated CBD and abnormal liver function tests (LFTs) generally merit noninvasive bile duct imaging by EUS; however, data are limited regarding the yield of such EUS in patients with CBD dilation and normal LFTs, who are unlikely to harbor significant pathology. [1231011121314] Furthermore, little is known about the predictive factors and natural history of asymptomatic CDL in patients with normal liver serum enzymes and CBD dilatation. [123] The purpose of this study is to determine the frequency and predictive factors of significant pathology in patients with CBD dilation and normal LFTs in whom EUS is requested.…”

“…However, fewer than 30% of patients are subjected to this procedure 1 , 7 , 8 , 14 , 15 , because either the disease is initially encountered in a locally advanced state or is metastatic, or because the patient is in poor clinical condition, making large-scale surgical procedures not feasible. Consequently, knowledge of palliative treatment is fundamental; this includes endoscopic, surgical, or radiological procedures or clinical therapeutic measures to relieve pain and obstructive jaundice and to clear duodenal obstruction 17 .…”

“…Palliative treatment of obstructive jaundice stands out because of the frequency of this condition 4 , 17 , 18 , 19 . Because of the morbidity related to obstructive jaundice, particularly the risk of development of cholangitis (inflammation of the biliary duct system, intrahepatic and extra-hepatic inflammation, or both) treatment is essential.…”
