Non-small-cell lung cancer (NSCLC) represents a very severe disease, being its incidence increasingly reported and, nowadays, successfully treatable only when surgery is deemed to be feasible. Furthermore, the disease and the clinical effects related to the complementary therapies (radio and/or chemotherapy) may strongly affect, frequently with dramatic clinical side effects, the patient’s ability to endure physical exercise. In such context, the PR(PR), which has already been proved to be useful and effective in other diseases such as COPD, could play a pivotal role. The aim of this review article is, therefore, to analyze the pertinent data recently reported in English literature in order to highlight the role of rehabilitation as complementary therapy in the management of patients with NSCLC. The evidence currently available suggests that, when surgery is indicated, PR is a safe and feasible option, both during pre-operative and post-operative timing.The safety and feasibility of rehabilitation are proven even in inoperable patients, although to date, little evidence has been reported on its role in the overall management of such complex diseases.

Postoperative pulmonary complications following thoracic surgery still have a high rate of occurrence, which increase the length of stay and also cause an increase in the rate of morbidity and mortality. Prevention and treatment of these complications are indications to pulmonary rehabilitation in preoperative and postoperative periods of thoracic surgery for promoting the patient to return his/her functional life and to being as active as possible. In this paper, considering the current literature and routine clinical knowledge, postoperative pulmonary rehabilitation in thoracic surgery will be mentioned and discussed. Despite the inadequate literature on this topic, it was proved that pulmonary rehabilitation programs including deep breathing exercises, coughing, ambulation, and pain control managements were effective. It was reported that, pulmonary rehabilitation programs including these methods reduced postoperative pulmonary complication rate, length of stay, need of medical service, and the cost of medical treatment. Considering the clinical benefits putting emphasis on to pulmonary rehabilitation which is simple, cost effective and safe would be important in surgery success, cost effectiveness and patient's outcomes.Keywords: Thoracic surgery, pulmonary rehabilitation, postoperative period ÖzetToraks cerrahilerinde halen yüksek oranda görülen postoperatif pulmoner komplikasyonlar hastaların hastanede kalış süresi uzatmakta, morbidite ve mortalite oranında artışa sebep olmaktadır. Bu komplikasyonların önlenmesi ve/veya sağaltımı, hastanın fonksiyonel yaşantısına aktif dönmesi için toraks cerrahilerinde preoperatif ve postoperatif dönemde pulmoner rehabilitasyon uygulama endikasyonu vardır. Bu yazıda; güncel literatür ve rutin klinik bilgiler doğrultusunda toraks cerrahisinde postoperatif pulmoner rehabilitasyon uygulamalarından bahsedilecektir. Konuyla ilgili yeterli sayı-da araştırma olmamasına rağmen, derin solunum egzersizleri, öksürme, ambulasyon ve ağrı kontrolünden oluşan pulmoner rehabilitasyon programlarının etkili olduğu kanıtlanmıştır. Bu yöntemlerden oluşan pulmoner rehabilitasyon programlarının, postoperatif pulmoner komplikasyon oranını, hastaların hastanede kalış süresini, sağlık hizmetlerinin kullanımını ve tedavi maliyetini düşürdüğü saptanmıştır. Klinik yararları göz önüne alınarak toraks cerrahi kliniklerinde basit, maliyeti düşük, riski düşük pulmoner rehabilitasyon uygulamalarına önem verilmesi, cerrahinin başarısı, sağlık maliyeti ve hasta kazançları açısından önemli olacaktır.Anahtar kelimeler: Toraks cerrahisi, pulmoner rehabilitasyon, postoperatif dönem GİRİŞSon 20 yılda postoperatif ağrı tedavisindeki geliş-meler, anestezi ve cerrahi alanındaki yenilikler, cerrahi sonrası genel komplikasyonları azaltmasına rağ-men postoperatif mortalite ve morbiditenin en önemli nedenini halen postoperatif pulmoner komplikasyonlar (PPK) oluşturmaktadır (1-3). A study showed that post-operative respiratory rehabilitation after lung resection for lung cancer was beneficial for the Borg dyspnea scale, exercise capacity represented by 6-minute walk distance, and maintenance of lung function (Cesario, et al 2007).
