Porphyria turcica. Twenty years after hexachlorobenzene intoxication

Abstract: Porphyria involving more than 4,000 persons occurred in Eastern Turkey from 1956 to 1961 due to ingestion of hexachlorobenzene (HCB), a fungicide added to wheat seedlings. During a site visit in 1977, 32 porphyric Turks (mean age, 29 years) were examined. Porphyrin concentrations were determined with samples of urine and stool from 29 porphyric and 10 control Turks and compared with results from 40 subjects in the United States. Distinctive clinical features were found in porphyric subjects, including hyperpig… Show more

“…Hexachlorobenzene (HCB) is an organochlorine compound that has been produced since 1933 (Barber et al 2005 ). It was widely used in the 1940s and 1950s as a fungicidal wood-preserving agent and to control wheat bunt (Cripps et al 1980 ; Gocmen et al 1986 ). At its peak, global HCB emissions have been estimated at up to 92,000 kg/year (Bailey 2001 ).…”

“…Specifically, very little information is available regarding health effects in infants following exposure to breast milk contaminated with pesticides. One welldocumented exception is the late 1950s incident in Eastern Turkey that involved about 4000 individuals poisoned by HCB that was added to wheat seedlings and processed into flour (Cripps et al, 1980(Cripps et al, , 1984Peters et al, 1982) . Many of the children under the age of 1 year who were breast -fed by mothers exposed to HCB ( i.e., high -level exposure) died from a disease known as pembe yara ( pink sore ) that was associated with weakness, convulsions, and annular erythema.…”

“…In a study 20-25 years after the epidemic, among 204 patients with a prior history of hexachlorobenzene-induced porphyria (Cripps et al 1980, Gocmen et al 1986) a picture of chronic disease was found which includes skin hyperpigmentation, hypertrichosis, scarring of the face and hands, painless arthritis, weakness, paraesthesia and myotonia. Less frequently, enlarged liver and thyroid and a recent history of red urine were found.…”
