Abstract: The coexistence of PCV with typical CSC-like active leaks and a thick choroid in the same eye simultaneously provides strong evidence that these diseases have an association and support the hypothesis that these conditions may originate from predisposed thick choroid.

“…Possible sequential processes of disease progression in the pachychoroid spectrum are PPE to CSC or PNV or PCV, CSC to PCV or PNV and PNV to PCV. [23][24][25][26] The time flow in this process is supportive of chronicity in eyes with PNV and PCV, which can explain the older age and higher numbers of pachydrusen in PCV and PNV. Other drusen such as reticular pseudodrusen and soft drusen may lead to neovascularization and/or atrophy.…”

“…Recent studies have suggested that pathogenesis of PCV may include a pachychoroid-driven process involving choroidal congestion, manifested by choroidal thickening and hyperpermeability 8 13 . Although the choroidal thickness is generally increased in eyes with PCV, a wide variation in choroidal thickness has been reported 14 16 .…”
