Introducción: la incidencia de infección por Pneumocystis jirovecii en pacientes con trasplante renal oscila entre 0.4-2.2%. Previo al uso de profilaxis, la incidencia era de hasta 14% en receptores de trasplante renal. El tratamiento de desensibilización previo al trasplante, dosis mayores de inhibidor de calcineurina, rechazo activo mediado por anticuerpos y un cuadro previo de infección por Pneumocystis jirovecii son los factores de riesgo más importantes para su presentación. Se describe un caso de receptor de trasplante renal con infección por Pneumocystis jirovecii y se discute tratamiento.
