Plantar Infection by Scopulariopsis brevicaulis

Abstract: A 42-year-old woman presented with a well-defined pruritic erythematous scaly plaque on the sole of each foot. The lesions, first noted about 15 years previously, were located in the medial plantar region and extended laterally. Mycological study revealed infection by Scopulariopsis brevicaulis. “…Most cases are associated with exposure to sandy beaches in tropical regions, where H. werneckii is found in areas of high salinity (588). However, individual cases due to Scopulariopsis brevicaulis, Phoma eupyrena, and Chaetomium globosum with findings consistent with tinea nigra have also been reported (57,154,287). Diagnosis is made by skin scraping, and biopsy is not needed.…”

“…Very little information exists about the in vitro activity of antifungals against Scopulariopsis. In the majority of reports, only one strain was tested and details about the technique used are generally scarce (1,2,4,5,8,9,11,13,14,20,21). In this study, in vitro antifungal susceptibilities of clinical and environmental isolates of Scopulariopsis spp.…”

“…Combined with itraconazole, amphotericin B showed good results on three occasions. Itraconazole was also used alone for one patient with ulcerous cheilitis (1) and one with plantar infection (5). The first patient relapsed after a certain degree of improvement, but the second was cured completely after the failure of ketoconazole and fluconazole.…”
