Plane xanthoma and multiple myeloma with lipoprotein--paraprotein complexing

Abstract: Clinicopathologic findings are reported of a woman with generalized plane xanthoma, multiple myeloma (IgG type K), and hyperlipemia with very high levels of serum cholesterol and triglyceride. Complexing of the serum lipoproteins and immunoglobulins had cryoglobulin properties and was separable by ultracentrifugation. Immunofluorescent studies of skin and bone marrow demonstrated deposits of IgG with low density lipoprotein apoprotein and IgG with beta-lipoprotein, respectively. Although immunosuppressive ther… Show more

“…However, Touton cells have been observed in HX and NX in several cases in the literature. 20,49,57,62,[78][79][80][81][82][83][84][85] Moreover, as demonstrated here, foam cells and Touton cells share phenotypic characteristics, including the surface expression of the CD163 antigen, which would suggest a common macrophagic origin. Finally, immune complex formation resulting from antibody activity of the MIg directed against various lipoproteins appears to represent a common physiopathologic pathway leading to a lipid accumulation in the macrophagic cells.…”

“…A predominant association of DPNX with paraproteinemia of light-chain immunoglobulins seems to occur with the 位 isotype prevailing. 8,9 As in our case, an extensive cutaneous involvement is in favor of an underlying systemic disease; multiple myeloma as well as other hematological, lymph proliferative disorders were ruled out and a MGUS was considered the An Bras Dermatol. 2011;86(4Supl1):S50-2.…”

“…Those complexes may be recognized as modified LDL by scavenger macrophages leading to development of cutaneous xanthomas. 7,8 For this reason, DPNX is considered a histiocytosisderived xanthomatosis (histiocytic lipidization). 5 Since the first time that the association between paraproteins and DPNX has been suggested, 21 cases with multiple myeloma and xanthomatosis have been confirmed as having lipoprotein-paraprotein complexes.…”

“…Cutaneous xanthoma occurring in association with paraproteinemia is a well-documented clinical abnormality that is seen in patients with multiple myeloma, benign monoclonal gammapathies, and other reticular endothelial disorders (1)(2)(3)(4)(5)(6). In many of these individuals the occurrence of xanthoma is associated with hyperlipidemia, but it should be recognized that cutaneous xanthoma in association with paraproteinemia can be observed in individuals whose plasma lipid levels are within the normal range (1,3,5,(7)(8)(9).…”
