Placement of Stereotactic Electroencephalography Depth Electrodes Using the Stealth Autoguide Robotic System: Technical Methods and Initial Results

Abstract: BACKGROUND:Robotic systems are gaining acceptance as a preferred tool for the placement of electrodes for stereotactic electroencephalography (SEEG) studies.OBJECTIVE:To describe the technical methods for insertion of SEEG using the Medtronic Stealth Autoguide robotic system and detailed outcomes in the initial 9 patients implanted.METHODS:Nine patients underwent placement of electrodes for SEEG studies with the use of the Autoguide system. Patients had at least 10 electrodes placed. Targets were planned on a … Show more

To the Editor:We appreciate the letter from Dr Philipp 1 in response to our article "Placement of Stereotactic Electroencephalography Depth Electrodes Using the Stealth Autoguide Robotic System: Technical Methods and Initial Results." 2 The authors are critical of our results, indicating that they do not support our conclusions. Rather, they suggest that the Autoguide system is both unsafe and inaccurate.

“…Fully automated systems continue to be the gold standard for placement of depth electrodes, deep brain stimulation, and other highly location-based neurosurgical treatments, although some groups are challenging this paradigm. [14][15][16] In this study, we sought to describe the surgical experience with the Autoguide over a consecutive series of patients.…”

“…Although we did not assess this in the current study, the minimally invasive access parameters of this robotic system could be applied to performing awake biopsy procedures and have been successfully applied to both laser interstitial thermal therapy 16 and electroencephalography lead placement. 14,15 Use of the robotic system for cranial lesion biopsy was highly efficient. The median total operative time was short, at 55.0 minutes, even accounting for frozen pathology results to be sent back to the OR and the addition of secondary procedures such as VP shunt placement.…”

“…The referenced article by Tay et al 1 describes a technique for implantation of depth electrodes for stereoelectroencephalography (sEEG) using the Stealth Autoguide system (Medtronic). This approach is a frameless technique, which uses a robotic arm for assisted placement of electrodes.…”
