Background: Healthcare systems around the world have begun to move towards a person (or patient)-centered care approach. Although this approach seems to have been newly adopted in some healthcare organizations, there is no evidence of person-centered care among hospitals in Saudi Arabia. The aims of this study were to assess patients’ perspectives on the climate of person-centered care and its associated factors in a Saudi tertiary hospital, and to examine the reliability of the Arabic version of the Person-centered Climate Questionnaire – Patient (PCQ-P) version. Methods: Using a cross-sectional study design, the validated version of the PCQ-P was distributed to 300 adult patients admitted to hospital for more than 48 hours. Patients from 16 inpatient departments at King Faisal Specialist Hospital and Research Center, a tertiary hospital in Riyadh, Saudi Arabia, were interviewed using the PCQ-P. Descriptive and inferential statistical analysis was performed using SPSS (version 22; IBM, NY, USA). Results: A total of 300 questionnaires were completed. Within this number of respondents, 159 (53%) were females; 119 were aged 21–40 years; 72.7% were married; 147 (49%) had attained up to a high school level of education; and (67.4%) were unemployed. Inpatients’ overall mean PCQ-P score was 73 ± 9.988 out of 85. Results suggested some significant associations between patient characteristics and their perspectives on person-centered care, such as age ( P =0.005), gender ( P <0.001), nationality ( P =0.026), area of residency ( P =0.001), route to admission ( P =0.002), length of stay ( P =0.003), and hospital preference ( P =0.010). The Arabic version of the PCQ-P was found to be reliable (Cronbach’s α=0.840), indicating its applicability to measure the climate of person-centered care in an Arabic-speaking context. Conclusions: Patient and hospital characteristics are important in terms of patients’ perceptions of the climate of person-centered care. The Arabic translation of the PCQ-P tool is a reliable way to measure this climate within the setting of an Arabic-speaking tertiary care hospital.

Purpose: To test the reliability and validity of the Korean version of the Person-centered Climate Questionnairestaff version (KPCQ-S) in long-term care institutions. Methods: A total of 297 staff in long-term care institutions including nine nursing homes (NHs) and 4 long-term care hospitals (LTCHs) were included. The KPCQ-S was developed following the WHO guidelines of the process of translation and adaptation of instruments. An internal consistency using Cronbach's ⍺ was tested for reliability. Exploratory factor analysis (EFA) was used to examine the construct validity. Convergent and discriminant validity were examined using Pearson correlation. Results: EFA demonstrated the construct validity of the 14-item KPCQ-S with three-factor solutions, specifically three factors (safety, everydayness, and community) in NHs and four factors (safety, everydayness, community, and comprehensibility) in LTCHs. Convergent validity was found in the correlation with the work satisfaction (r=.55). The KPCQ-S showed satisfactory internal consistency reliability (Cronbach's ⍺=.91). Conclusion: The KPCQ-S is found to be a reliable and valid tool for measuring staff perceptions of the person centeredness of long-term care environments.

The article by Morris and colleagues in this issue of JAMDA, entitled "Hearing the Voice of the Resident in Long-Term Care Facilities-an Internationally Based Approach to Assessing Quality of Life," 1 underscores the importance of including residents' perspectives regarding the care they receive. InterRAI, an international research collaborative, developed a Self-Report Quality of Life Survey for Long-Term Care Facilities (SQOL-LTCF) which assesses resident perspectives on subscales reflecting social life, personal control, food, caring staff, and staff responsiveness. The results are based on reports of more than 16,000 residents who resided in 355 LTCFs; 44% of the respondents are from Belgium (residing in 70% of the LTCFs in the study), 32% are from Canada, 21% are from the United States, and fewer than 1% are from each of Poland, Estonia, South Africa, the Czech Republic, and Australia. Based on the distributions of the data, the researchers collapsed the scores (i.e., never/rarely; sometimes; most of the time; always); established benchmark standards for subscales; and compared scores to a single item (home-likeness) that was considered to represent an overall measure of "personal quality of life." This study is valuable in its findings as well as in the issues it raises inherent to cross-cultural research: with long-term care a reality around the world, to what extent is research on care systems broadly generalizable? Obtaining perspectives directly from residents is important, and since 2010 has been a standard part of screening in the U.S. Minimum Data Set 3.0 Resident Assessment Instrument (MDS 3.0 RAI) for nursing homes. 2,3 However, not all residents are able to self-report, and in the interRAI effort, those with more severe cognitive impairment were excluded from participation.
