Persistent Parathyroid Hormone Elevation Following Curative Parathyroidectomy for Primary Hyperparathyroidism
Abstract: Persistent PTH elevation occurs in 27% of patients following curative parathyroidectomy and is usually a transient phenomenon more common in patients with musculoskeletal symptoms and of the black race. It is not a manifestation of persistent disease but is most likely a secondary response to bone remineralization.

“…Patients with postoperative NPE had severe parathyroid disease with larger adenomas and high preoperative levels of serum calcium and iPTH. Our results were consistent to previous findings [ 3 , 7 , 10 , 13 , 16 ]. Beyer et al reported that patients routinely receiving oral calcium and vitamin D supplementation after parathyroidectomy reduced the incidence of NPE from 37% to 12% [ 10 ].…”

“…This is consistent with prior reports showing this phenomenon occurs in 8% to 40% of patients after curative parathyroidectomy . Some of these studies have suggested that persistently, ePTH is more likely to occur in elderly patients or African American patients . We found no significant differences in these variables between patients with elevated and normal PTH levels postoperatively, perhaps owing to regional differences in patient populations.…”
