“…One limitation of transcatheter pulmonary valve implantation is the need for a proper landing zone for anchoring, which may not be present in the large native RVOT (5). Other limitations include issues with positioning the THV, endocarditis, and paravalvular regurgitation (5). The present case report shows how this is safe and efficient in carefully selected patients.…”

“…This percutaneous approach has several advantages compared to surgery, which implies repeated sternotomy, cardiopulmonary bypass, and a longer recovery time. One limitation of transcatheter pulmonary valve implantation is the need for a proper landing zone for anchoring, which may not be present in the large native RVOT (5). Other limitations include issues with positioning the THV, endocarditis, and paravalvular regurgitation (5).…”

“…In RVOTs >25 mm, sizing can be performed by occluding the RVOT with a large (eg 30mm) balloon and infusing contrast in the RV while executing a push-and-pull manoeuvre to ensure a stable position before stent implantation. In patients with native RVOTs, PPVI can be attempted in RVOT diameters <28 mm measured with balloon testing [ 28 ], but pre-stenting may be particularly challenging in these cases. Initially, valve implantation was delayed up to several months after pre-stenting ensuring good stent ingrowth and stable position.…”

“…Wird die Klappeninsuffizienz rechtzeitig behandelt, kann das rechtsventrikuläre Volumen wieder abnehmen und die Leistungsfähigkeit steigen [18]. Allerdings ist dieser Effekt in vielen Fällen relativ gering, sodass die Frage offenbleibt, ob die Behandlung einer Pulmonalklappeninsuffizienz noch früher als bisher beginnen sollte, wenn dies per Katheter minimalinvasiv möglich ist.…”
