Percutaneous edge‐to‐edge mitral valve repair: 2‐year follow‐up in the first human case

Abstract: We report the first-in-human mitral valve repair using percutaneous technology that creates a "surgical-like" edge-to-edge repair. A catheter-delivered clip was introduced transseptally from the femoral vein, advanced through the mitral orifice, retracted to grasp the leaflets, and detached to create a functional double-orifice valve. The patient had an uncomplicated post-procedural course. Echocardiography at 1- and 2-years post procedure showed mild mitral regurgitation and positive ventricular remodeling. T… Show more

“…The first human implant of a MitraClip was performed in June 2003 by Dr. Jose Condado in Caracas Venezuela in a 48-year-old woman with severe MR due to a bileaflet flail. The procedure was performed without complications and after successful clip deployment her MR decreased to <2+ [26]. Most importantly, her symptoms resolved after the procedure and MR has remained mild on echocardiographic follow-up over a period of seven years.…”

“…Two years after the procedure, the patient showed mild residual mitral regurgitation and favorable reverse remodeling [14]. Soon after the first in man experience, the Endovascular Valve Edge-to-Edge REpair Study (EVEREST) was initiated in the United States.…”

“…These techniques are based on two general principles. One uses the Alfi eri approach to appose the two mitral leafl ets [39]. The other takes advantage of the sometimes close approximation of the coronary sinus to the mitral annulus, instrumenting the coronary sinus to reduce its diameter, thereby also reducing the mitral annulus [40].…”
