Parathyroid Reimplantation in Forearm Subcutaneous Tissue During Thyroidectomy: A Simple and Effective Way to Avoid Hypoparathyroidism

Abstract: Parathyroid gland reimplantation in forearm subcutaneous tissue during thyroid surgery is a safe, easy and effective procedure; furthermore, it allows a good control of graft functionality and would allow an easy grafted gland removal if needed.

“…In the study of El-Sharaky et al, function of parathyroid graft was assessed one month after surgery but not at one year (38). A recent prospective study on parathyroid forearm reimplantation (40), documented a gradient (range x1.5-5) between grafted and non-grafted forearms at three months, but failed to properly document the long-term parathyroid function. In addition, the authors report only 2/25 cases of postoperative hypocalcaemia, at variance with almost all studies.…”

“…Maintaining intact parathyroids in place is the most important factor in preventing hypoparathyroidism (44,55). This requires from the surgeons a broad knowledge of the anatomy and embryology of the parathyroids, meticulous surgical technique, and command of parathyroid autotransplantation techniques in case the glands are removed (43,56,57).…”

“…Although auto-transplantation of the parathyroid glands is reportedly effective for long-term recovery of parathyroid gland function [17,18], the post-recovery parathyroid gland hormone levels were much lower with auto-transplantation of the parathyroid glands when there was no preserved parathyroid gland compared with auto-transplantation of the parathyroid glands when there was at least one preserved parathyroid gland [19]. …”
