Purpose To evaluate with optical coherence tomography angiography the status of the choriocapillaris in the eyes of patients with resolved paracentral acute middle maculopathy (PAMM). Methods Nine patients with unilateral resolved PAMM, 14 age-matched healthy individuals, and 16 age-matched patients with unilateral retinal vein occlusion (RVO) were included. The choriocapillaris flow area, and the number of choriocapillaris flow voids was compared between the eyes of healthy individuals, both eyes of the PAMM patients, and the fellow eyes of unilateral RVO patients. Results The number of choriocapillaris flow voids in the unaffected eyes of PAMM patients was statistically significantly higher than in the eyes of healthy individuals (p < 0.001) and in the fellow eyes of unilateral RVO patients (p = 0.022). The choriocapillaris flow area in the unaffected eyes of PAMM patients was statistically significantly lower than in the eyes of healthy individuals (p < 0.001) and in the fellow eyes of unilateral RVO patients (p = 0.019). There was no difference in the status of the choriocapillaris between two eyes of PAMM patients. Conclusion Eyes of patients with resolved PAMM have a substantial decrease of choriocapillaris perfusion.

Paracentral acute middle maculopathy represents deep capillary ischemia that can cause acute symptoms of scotoma. This case series describes two cases of symptomatic paracentral acute middle maculopathy that reflect the more severe spectrum of associated etiologies. Case 1: A 65-year-old male with recent history of hypertensive encephalopathy presented with sudden onset central visual disturbance described as amorphic in shape. Dilated fundus exam revealed severe hypertensive retinopathy with paracentral acute middle maculopathy. The patient died one year after the initial discovery of the retinal lesions. Case 2: A 87-year-old male presented with paracentral vision loss in one eye for three days. Clinical examination revealed multiple Hollenhorst plaques and paracentral acute middle maculopathy at presentation, and a subsequent branch retinal artery occlusion two days later. The patient was found to have 76% left carotid artery stenosis with numerous brain infarcts. The detection of paracentral acute middle maculopathy indicates an investigation through ocular and systemic risk factors to determine an attributable etiology. The search for possible etiologies should be tailored to the particular patient, based on age, associated ocular findings, systemic history, and symptomatic versus incidental presentation. The presence of symptomatic paracentral acute middle maculopathy, particularly in at-risk vasculopathic patients, should be addressed urgently as it may be an indicator of an underlying severe systemic disease process. Purpose: To describe a representative case and review the literature on paracentral acute middle maculopathy (PAMM) and giant cell arteritis (GCA).Methods: A review of the English-language ophthalmic literature was performed using the search terms of PAMM, giant cell arteritis, and temporal arteritis.Results: A 72-year-old woman with PAMM as the presenting ophthalmic manifestation of GCA was described with a review of the prior cases from the literature. It was found that there were 26 cases of PAMM in GCA. In 19 of 26 cases, PAMM was associated with no other fundus abnormalities and was only seen on multimodal imaging including optical coherence tomography.Conclusion: PAMM can cause acute paracentral visual loss, and GCA should be suspected in all cases of PAMM of the elderly patients, even when isolated and not associated with constitutional symptoms of GCA.
