Pain Management in Childhood Leukemia: Diagnosis and Available Analgesic Treatments

Abstract: Pain is one of the most common symptoms in children suffering from leukemia, who are often misdiagnosed with other childhood painful diseases such as juvenile idiopathic arthritis. Corticosteroid-induced osteonecrosis (ON) and vincristine-induced peripheral neuropathy (VIPN) are the most common painful manifestations. Additionally, ongoing pain may continue to impact quality of life in survivorship. “…Acute lymphoblastic leukemia (ALL) is the most common neoplasm in children, accounting for about 30 percent of all pediatric malignancies (Coluzzi et al, 2020). High-dose methotrexate (HDMTX) is commonly defined as an intravenous dose greater than 500 mg/m 2 (Howard et al, 2016), and HDMTX is recommended as an essential component of chemotherapy for ALL and non-Hodgkin lymphoma (NHL) in clinical guidelines (National Comprehensive Cancer Network, 2021a;.…”

“…Adequate pain management in children with ALL is an indispensable supportive care tool. In these patients, pain often occurs in connection with osteonecrosis, peripheral neuropathy caused by vincristine or mucosal or gastrointestinal side-effects (e.g., mucositis, enteritis, obstipation) and must be administered in the correct dose for the respective indication, regardless of the substances' possible off-label use in children ( 113 ). Going into detail about pain medication in pediatric oncology would exceed the scope of this review.…”
