48-78 % of patients admitted to surgical hospitals with ventral hernias are obese, which has a significant impact on the course of the disease and the postoperative period, and little is reported in the literature. Obesity, on the one hand, contributes to the formation of hernia, on the otherprogresses with its appearance. Despite the large number of surgical procedures to correct the abdominal wall, the results of surgical treatment of primary and postoperative ventral hernias, especially in morbid obesity are unsatisfactory, due to errors in choosing the optimal method of surgery indicated by the high number of complications and high recurrence. Recurrences after surgical treatment of primary and postoperative ventral hernias occur in 10-60 % of cases, which indicates that the problem of surgical restoration of the abdominal wall is still far from being solved. Objective. Conduct a retrospective analysis of the results of surgical treatment of patients with ventral and postoperative ventral hernia to determine the main risk factors for complications in patients with morbid obesity. Materials and methods. The results of surgical treatment of 183 patients operated on for primary and postoperative ventral hernia with concomitant morbid obesity in 2015-2020 combination with dermatolipectomy and liposuction. A total of 46 (25.14 %) were performed. Dermatolipectomies were performed by Kuster, Jolly, Thorek and Castansres-Goethel. Pulmonary artery thromboembolism (PE) was diagnosed in 2 (1.09 %) surgically operated: 1 (1.31 %) patient with degree I of obesity and 1 (1.72%) with degree II respectively. Abdominal compartment syndrome (ACS) was diagnosed in 4 (2.19 %) surgically operated: among them with obesity degree IIin 2 (3.45 %) patients and degree IIIin 2 (4.08 %) patients. Significantly higher frequency of wound complications was observed in patients with obesity degree II-15 (26.17 %) and degree III-16 (31.41 %). Hernia recurrence occurred in 10 (12.58 %) surgically operated with obesity degree I, in 6 (9.33 %) with degree II, and in 6 (11.82 %) with degree III. Conclusions. Performance of laparoscopic and non-tension implantation methods of hernioplasty should be a priority, especially in groups of patients with pre-existing morbid obesity degree II-III. In obese patients with degree III there is a significantly higher incidence 191 of abdominal compartment syndrome, and the simultaneous dermatolipectomy or liposuction significantly reduces the incidence of local complications and recurrences.

A systematic review of a recently published meta-analysis shows that laparoscopic and open allogeneoplasty is a safe procedure with relatively short-term and long-term results. A new minimally invasive technique has been developed that allows the mesh to be placed in the retroperitoneal space through a small transhernial incision, avoiding signiﬁ cant trauma to the abdominal wall and contact with the abdominal cavity. However, this type of operation does not involve ﬁ xation of the mesh in the retromuscular space, which can lead to a number of complications. The analysis of the obtained results determines the relevance of this study.Objective. To compare the features of the ultrastructural reaction of the tissues of the muscle- aponeurotic layer of tissues to the implantation of «Capromesh» in combination with PRP and polypropylene meshes in an experiment.Material and methods. The study was conducted on 16 same-sex pigs of the Vietnamese breed weighing at least 10 kg, which were divided into 2 groups. Histological and ultrastructural changes in the tissues of the muscle- aponeurotic layer of the anterior abdominal wall after implantation in the retromuscular space of «Capromesh» in combination with PRP (plasma enriched with growth factors) and polypropylene meshes were studied in the experiment.The results. A signiﬁ cant diﬀ erence in tissue reactions at diﬀ erent times of the experiment on the implanted material was proven. The conducted microscopic and electron microscopic studies after the implantation of the «Capromesh» mesh in combination with PRP established that starting from the 7th day of the experiment, the inﬂ ammatory changes in the tissues of the muscle- aponeurotic layer are not as signiﬁ cant as during the implantation of the polypropylene mesh. Starting from the 14th day of the experiment, the signs characterizing the reduction of the inﬂ ammatory reaction and the more intense formation of ﬁ brous structures around the mesh material are much better expressed. On the 21st day of the experiment, at the microscopic and electron microscopic levels, a signiﬁ cantly smaller number of areas of leukocyte inﬁ ltration, intensive vascularization from the formation of collagen ﬁ bers with the participation of ﬁ broblasts around the «Capromesh» mesh material in combination with PRP was established.Conclusions. The conducted micro- and ultrastructural studies determined the priority of using the «Capromesh» mesh in combination with PRP under the conditions of material selection when performing retromuscular allogeneoplasty.

Алопластика є «золотим стандартом» в лікуванні гриж живота, проте значну проблему становлять місцеві післяопераційні ускладнення пов’язані з сіткою, такі як серома, інфікування, нориці передньої черевної стінки та рецидив грижі. Частота виникнення післяопераційних ускладнень після алопластики залежить від багатьох факторів, які пов’язані зі способом життя пацієнтів, супутніми патологіями, видом, локалізацією грижі та методиками операцій. Мета. Вивчити основні причини та фактори ризику виникнення ускладнень після алогерніопластики шляхом порівняльного аналізу клініко-анамнестичних особливостей пацієнтів. Матеріали і методи. Проведено ретроспективний аналіз медичної документації 192 пацієнтів з післяопераційними ускладненнями після алопластики гриж живота. І групу (n=86) склали пацієнти з рецидивними грижами живота поєднаними з норицями передньої черевної стінки, ІІ групу (n=106) – пацієнти з глибокими інфікованими ранами з залученням сітчастого імплантату після алопластики грижі живота. Результати. У І групі серед основних причин були виявлені серома у 60,5% та інфікування рани у ранньому післяопераційному періоді у 22,1%, у ІІ групі первинне інфікування рани у 38,7%, тривала серома, що нагноїлась у 33,0% пацієнтів. В залежності від методики: методику onlay виконували у 59,3% пацієнтів І групи та 70,8% ІІ групи, sublay у 27,9% пацієнтів І групи та у 29,2% ІІ групи. Не виявлено різниці між факторами ризику та супутньою патологією у І та ІІ групах та порівняно з літературними даними. Висновки. Основними причинами виникнення глибоких інфікованих ран з залученням сітчастого імплантату та рецидивів гриж живота поєднаних з норицями передньої черевної стінки після алогерніопластики були тривала серома, що інфікувалась та нагноєння рани в ранньому післяопераційному періоді, які виникали здебільшого після алопластики за методикою оnlay, що обґрунтовувало профілактику виникнення сером та інфікування рани з метою попередження незадовільних результатів алопластики.
