Outcomes of endoscopic ultrasound‐guided pancreatic FNAC diagnosis for solid and cystic lesions at Manchester Royal Infirmary based upon the Papanicolaou Society of Cytopathology pancreaticobiliary terminology classification scheme

Abstract: Results: 120 EUS-FNA pancreas specimens from 111 patients were received, of which 112 (93.3%) specimens had follow-up data. There were 79 and 41 EUS-FNA pancreas specimens from solid and cystic lesions, respectively. Based on the cytology diagnosis the specimens were classified as Panc 1 (7.5%), Panc 2 (33.3%), Panc 3 (2.5%), Panc 4B (2.5%), Panc 4O (15.0%), Panc 5 (3.3%) and Panc 6 (35.9%). The performance indicators for diagnosis of malignancy or neoplasia with malignant potential, included sensitivity (95.4… Show more

“…One of the most egregious results in the study was the high rate of inadequacy in specimens. The nondiagnostic rate of 21.2% is higher than rates reported by other authors, ranging from 3.8%‐11.7% 9‐12 . The explanation is multifactorial.…”

“…In spite of an accurate correlation in these cases, an attempt to use these data to suggest a high accuracy of pancreatic FNA involving all the PSC categories would be inappropriate based on the current study because of a low proportion of cases with available follow‐up. Wright et al 19 used PSC to categorise 120 samples. Although they had histopathology results in only 20 of these cases, clinical follow‐up data were available in 112 of cases.…”

“…The distribution of the cases using the PB system from studies looking at pancreatic FNA specimens places most cases in the negative for malignancy and malignant categories [2][3][4][5][6][7][8][9] (Table 1). In fact, one of the clear benefits of implementing the PB System is the ability to decrease the number of indeterminate interpretations (eg, atypical and nondiagnostic).…”
