In order to develop evidence-based recommendations and expert consensus for nutrition management of patients undergoing bariatric surgery and postoperative follow-up, we conducted a systematic literature search using PRISMA methodology plus critical appraisal following the SIGN and AGREE-II procedures. The results were discussed among all members of the GARIN group, and all members answered a Likert scale questionnaire to assess the degree of support for every recommendation. Patients undergoing bariatric surgery should be screened preoperatively for some micronutrient deficiencies and treated accordingly. A VLCD (Very Low-Calorie Diet) should be used for 4–8 weeks prior to surgery. Postoperatively, a liquid diet should be maintained for a month, followed by a semi-solid diet also for one month. Protein requirements (1–1.5 g/kg) should be estimated using adjusted weight. Systematic use of specific multivitamin supplements is encouraged. Calcium citrate and vitamin D supplements should be used at higher doses than are currently recommended. The use of proton-pump inhibitors should be individualised, and vitamin B12 and iron should be supplemented in case of deficit. All patients, especially pregnant women, teenagers, and elderly patients require a multidisciplinary approach and specialised follow-up. These recommendations and suggestions regarding nutrition management when undergoing bariatric surgery and postoperative follow-up have direct clinical applicability.

Aim: Although bariatric surgery for elderly obese patients is having high complication risks and with indefinite results, can be still beneficifial. The aim of this study is to evaluate the results of sleeve gastrectomy in elderly patients.Material and Method: All 65 years or older patients underwent laparascopic sleeve gastrectomy operation between April 2013-September 2017 included in this retrospective study. Demographic data, comorbidities due to obesity, body mass index, pre and post operative weight, recovery from comorbidities, followup time, postoperative complications, and duration of hospital stay were recorded. Elderly patients and patients under 65 years underwent sleeve gastrectomy were comparised according to results.Results: Fourteen patients (mean age 66.9±1.9 years) operated on for laparascopic sleeve gastrectomy. Mean 52.9% BMI loss obtained in the patients after 19-month follow-up. In the elderly patients, significant improvements were obtained for the comorbidities due to obesity. Complication rates were similiar among the two groups. Conclusion:Laparascopic sleeve gastrectomy is an effecitive and safe operation in elderly obese patients, but weight loss is more tolerable compared to younger patients. A selective group of elderly patients may benefit from bariatric surgery.

Bariatric Surgery Impact on Cardiovascular Risk Factors: Is Age a Factor to Consider?

Introduction: Despite the abundance of data addressing the influence of patient’s age on surgery-related complications, its impact on cardiometabolic outcomes following bariatric surgery has been overlooked. Methods: Retrospective unicentric study of 1,728 obese patients who underwent bariatric surgery between January 2010 and June 2015. Patients were divided in 3 age groups, according to their age at surgery: ˂40 (n = 751), 40–59 (n = 879), and ≥60 years (n = 98). Parameters with cardiometabolic impact, such as body anthropometric measures, lipid profile, and glycemic status, before and 24 months after surgery, were compared between these groups. A multiple linear regression was performed, adjusting differences between groups for sex, surgery type, and body mass index variation. Results: The group ˂40 years presented more weight loss (–35.4 ± 9.0 kg, p ˂ 0.001), greater BMI reduction (–15.8 ± 6.1 kg/m2, p ˂ 0.001), and larger changes in waist (–34 ± 13.8 cm, p ˂ 0.001) and hip circumferences (–28.7 ± 11.9 cm, p ˂ 0.05). The group of ≥60 years presented the heaviest reduction in fasting glucose (–17.7 ± 32.8 mg/dL, p ˂ 0.001) and HbA1c (0.7 ± 1.0, p ˂ 0.001), and also had a tendency to have the biggest changes in systolic blood pressure (–14.7 ± 18.7 mm Hg, p = 0.071). Conclusion: Patients with ≥60 years benefit the most from bariatric surgery regarding cardiometabolic parameters, presenting heavier reductions in fasting glucose, as well as HbA1c and a tendency towards a higher decrease in systolic blood pressure. No clinically significant differences in lipid profile were observed between groups.
