Background:The aim of this study was to investigate the relationship among management modality, glycemic control, components of metabolic syndrome (MS) and serum levels of γ glutamyl transferase (GGT) and C-reactive protein (CRP) in patients with type 2 diabetes (T2DM). Methods: Patients with T2DM, not suffering from diabetes complications, were recruited from outpatients clinics at two hospitals in Jeddah. Anthropometric measurements and blood pressure (BP) were taken. A treatment plan was recorded. Fasting blood samples were obtained to measure glucose, glycated hemoglobin (HbA1c), lipids profile, highly sensitive (hs)-CRP and GGT. Results: A total of 71 men and 82 women were recruited. Lower mean HbA1c was found in people receiving oral glucose-lowering drugs compared with those on insulin therapy (p < 0.001). Management modality had no effect on mean GGT or hs-CRP. Higher mean GGT was associated with poor glycemic control, dyslipidemia, hypertension, and abdominal obesity. GGT correlated significantly (p < 0.05) and directly with triglycerides in men (r = 0.401) and diastolic BP (r = 0.279 for men, r = 0.194, for women), but inversely with high-density lipoprotein cholesterol (HDL-C) (r = -0.298 for men, r = -0.171 for women). hs-CRP correlated with waist circumference (p < 0.05, r = 0.312, for men, r = 0.305, for women), with a higher mean being found in men with poor glycemic control (p = 0.015), in hypertensive women (p = 0.030), and in patients who were abdominally obese (p < 0.05). Conclusions: High levels of GGT and hs-CRP are associated with components of MS and poor glycemic control, hence increased cardiovascular risk. Due to their value as independent risk predictors of vascular injury, these measures should be included in routine monitoring of patients with T2DM.

Öz amaÇ: Çalışmamızın amacı, böbrek nakli yaptığımız işlevsel grefti olan hastalarda osteoporoz sıklığını ve risk etmenlerini belirlemektir. gErEÇ ve YÖntEMlEr: 1997-2010 yılları arasında böbrek nakli yapılan, nakil sonrası kemik mineral yoğunluğu (KMY) ölçümü olan 92 hasta ile çalışmamız yapıldı.BulgulAr: 92 hastamızın 57'si (%62) erkek, 35'i (%38) kadın idi. Hastaların yaş ortalaması 36 yıldı (19-62 yıl). Femur ve/veya lomber vertebra KMY sonuçlarına göre; 39 (%42,4) hasta normal, 39 (%42,4) hastada osteopeni ve 14 (%15,2) hastada osteoporoz vardı. KMY sonuçları ile yaşın, cinsiyetin, vücut kitle indeksinin, paratiroidektomi durumunun, toplam steroid ve bağışıklığı baskılayıcı ilaç dozunun, KMY ölçümü sırasındaki serum kreatinin, kalsiyum, fosfor, alkalen fosfataz düzeylerinin arasında anlamlı ilişki yoktu (p>0,05).SOnuÇ: Bizim çalışmamızda, risk faktörlerin hiçbirisi ile KMY arasında anlamlı bir ilişki saptanmadı. Bu bulgular; tek başına KMY ölçümünün böbrek naklinden sonra gelişmesi olası kemik hastalığını izlemede iyi bir gösterge olmadığını göstermektedir. Bu konuda pratik yeni yöntemlere gereksinim vardır.AnAhtAr SÖzCüKlEr: Kronik böbrek hastalığı, Böbrek nakli, Kemik mineral yoğunluğu abstract OBJECtIVE: The aim of our study was to determine the frequency and risk factors of osteoporosis in patients who had undergone renal transplantation in our clinic and had a functional graft. MAtErIAl and MEthODS:This study included 92 patients who had undergone renal transplantation between 1997 and 2010 and whose bone mineral density was measured after transplantation. rESultS: Our study included 92 patients. 57 patients (62%) were male and 35 (38%) were female. The mean age of the patients was 36 years (range: 19-62 years). According to the bone mineral density in the femur and/or lumbar vertebrae, 39 (42.4%) patients were normal, 39 (42.4%) had osteopenia and 14 (15.2%) had osteoporosis. There was no statistical relationship between the patients' age, gender, body mass index, history of parathyroidectomy, total steroid and immunosuppressive drug dose, creatininecalcium-phosphorus-alkaline phosphatase level at the time of the bone mineral density measurement and bone mineral density (p>0,05). COnCluSIOn:We did not find any relationship between the above risk factors and bone mineral density. These findings show that bone mineral density is not a good parameter in the follow-up of bone disease after renal transplantation.

Cancer patients are at risk for adverse events involving bone. Metastasis of cancer to bone and primary bone tumors can compromise the integrity of bone. Various cancer therapies cause long-term skeletal disorders, particularly bone loss, osteomalacia, and avascular necrosis. Cancer therapies that include chemotherapy, glucocorticoids, hormonal agents, and newer targeted therapies can affect bone in several ways. With the improved effectiveness of cancer treatment, more cancer patients are surviving longer and may experience fractures as a long-term complication of bone loss. Prevention of bone loss through early detection and appropriate use of anti-osteoporosis treatment may decrease bone loss and fractures. This article reviews causative risk factors, mechanisms, and prevention and treatment strategies for cancer therapy-related bone loss in hematologic and specific solid malignancies.
