Background. Oro-dental diseases are prevalent in chronic liver failure (CLF) patients. The aim of this study was to evaluate the quality of life associated with oral health in candidates for liver transplant surgery. Materials and Methods. The demographic information of 105 end-stage liver cirrhotic patients was collected. All patients were ordered a panoramic view for pretransplant dental evaluation. The DMFT (decayed-missing-filled tooth) index was calculated for dental examination. The model for end-stage liver disease (MELD) was used for the severity of liver disease. The OHIP-14 (Oral Health Impact Profile) questionnaire and GOHAI (Geriatric Oral Health Assessment Index) questionnaire were applied to evaluate the impact of oral disease on the quality of life. Results. A total of 79 patients thoroughly completed the questionnaires; 79.7% were male, 32.9% were over 50, and 25.3% were less than 30 years old. Further, 12.7% smoked, 2.5% were illiterate, 64.6% had not finished school, and 10.1% had university degrees. Almost half of the cirrhotic patients were suffering from the disease for more than 3 years. Most complaints reported by the patients as “very often” were becoming self-conscious (13.9%) and being uncomfortable when eating any foods (13.9%) followed by feeling tense (12.8%). There was no significant difference between gender, smoking, age, and MELD score based on quality of life (OHIP and GOHAI) ( P > 0.05 ). The level of education ( P = 0.020 ), duration of disease ( P = 0.017 ), and DMFT index ( P = 0.039 ) had a significant impact on oral health-related quality of life in CLF patients. An inverse relationship was seen between the DMFT index and the quality of life. Conclusion. Oral health has a high impact on the quality of life of cirrhotic patients. The psychological dimension of oral health is the most debilitating factor affecting the quality of life. This shows the importance of professional oral care, oral health, and self-care education in this group of patients.

A radiografia panorâmica é usada para a avaliação do estado dos dentes e osso dos maxilares, em uma visão global dessas estruturas de uma só vez. O objetivo deste estudo é caracterizar os pacientes e levantar aspectos radiográficos relevantes observados nas radiografias panorâmicas dos pacientes do Programa de Extensão Assistência Odontológica a Pacientes de Transplante da UFMG. Foram estudadas radiografias panorâmicas desde 2016. A análise estatística se deu por descrição dos resultados e análises bivariadas, empregando-se o teste do qui-quadrado e teste exato de Fischer, Mann-Whitney e Kruskal-Wallis. Foram considerados significativos os resultados com p< 0,05. Em relação à faixa etária, foi observada uma associação significativa com a avaliação da raiz residual, nível ósseo e relação da cúpula alveolar. A radiografia panorâmica contribui para a compreensão de problemas, acelerando o estabelecimento do plano de tratamento, especialmente entre pacientes que estejam na fase pré-transplante de fígado, rins ou células hematopoiéticas.

Background: Osteoporosis is a disease of the bone structure which has many outcomes for the patient. Tooth loss and failure of implant placement can be related to this disorder in the jaw bones which is shown by the reduction of mandibular inferior cortical thickness. The aim of this study is to find a relationship between mandibular inferior cortical thickness using panoramic radiographs and tooth existence. Methods: A total of 57 panoramic views of complete edentulous patients and 164 partial edentulous cases were evaluated and compared to 117 dentate age and sex matched subjects. Mandibular cortical thickness was measured on 3 reference points (below the mental foramen (S1), estimated position of the first molar (S2) estimated position of the third molar (S3)) on both left and right sides using marking gauge in Agfa program. Results: Data revealed that cortical bone thickness was significantly reduced in older patients (P=0.031). The final value of the three measurements of the mandibular border thickness was 2.3128mm ± 0.74840 in the right side (RS) and 2.407±0.802 mm in the left side (LS) for the complete edentulous group, 2.9026± 0.7513 mm in RS and 2.9976 ±1.06769 in LS for partial edentulous group and 2.8709± 0.6263 in RS and 2.9812 ±0.83081 in LS for the dentate cases. Complete edentulous cases and partial edentulous cases that had lost posterior teeth had no significant difference in cortical thickness, but both groups had a significant thinner cortical width compared to dentate subjects (P= 0.001 and P=0.002). But lack of anterior teeth did not have the same effect compared to dentate individuals (P=0.929). Conclusions: The present study demonstrates a significant relationship between tooth loss in the posterior region and reduction of inferior mandibular border which is a value of osteoporosis. Anterior tooth loss however does not have the same effect. Dentist and physicians should be in close contact in such patients to avoid further tooth loss and also diagnose this chronic disease in earlier stages.
