Osteochondral Autologous Transplantation Versus Dorsal Closing Wedge Metatarsal Osteotomy for the Treatment of Freiberg Infraction in Athletes: A Randomized Controlled Study With 3-Year Follow-up

Abstract: Background: Dorsiflexion closing wedge metatarsal osteotomy (DCWMO) has been considered the traditional treatment of Freiberg disease. Several case reports presented osteochondral autologous transplantation (OAT) as an alternative treatment. Purpose/Hypothesis: The purpose was to compare the results of DCWMO versus OAT for the treatment of Freiberg infraction in an athletic population. It was hypothesized that OAT was superior to DCWMO regarding functional outcomes, pain, and the time that the athletes returne… Show more

“…Since then, several techniques and fixation methods have been reported related to metatarsal osteotomy in Freiberg disease. 9,12,13,20 Known complications in patients with osteotomy include fixation failure and metatarsalgia. 1,23,28 In a study by Ikoma et al, 13 13 patients with Freiberg disease had extraarticular osteotomy, and after a follow-up period of a mean 17 months, dorsiflexion was seen to have increased from a mean of 37.2° preoperatively to 73.6°.…”

“…Another surgical option is osteochondral transplantation, and although good results have been reported, especially in athletes, donor site morbidity is known to be a complication. 9,14,24 One of the main disadvantages in retrospective studies is the loss of data. However, we have only 3 missing patients at the final follow-up.…”

“…Several previous studies pointed out the donor-site morbidity in the knee joint after OAT procedure. 1,11,16,18,20 We also have 1 patient with mild knee pain. The patient was a 51-year-old woman.…”

“…6 If conservative measures over several months fail, operative management may be necessary. 11 In 1979, Gauthier and Elbaz introduced dorsiflexion closing wedge metatarsal osteotomy (DCWMO). 9 Since then, several small series have reported good results with this procedure.…”
