Multiple myeloma (MM) is a monoclonal gammopathy, also known as the malignant proliferation of plasma cells, presenting with typical complications such as hypercalcemia, osteolytic bone lesions, anemia, renal insufficiency, and frequent infections. Central nervous system (CNS) involvement in MM in the form of parenchymal involvement is very uncommon and has been reported only in 1% of patients. CNS involvement as an initial presentation is very rare and has poor prognosis. Also specific treatment guidelines do not exist for the treatment of such patients. We present here a case of a 40-year-old female patient, with complaints of headache and left eye proptosis followed by left-sided hemiparesis, who after complete workup was diagnosed as a de novo case of stage III extramedullary MM with CNS involvement. Patient was treated with whole brain radiation followed by BCD regimen (injection of bortezomib, injection of cyclophosphamide, and tablet dexamethasone). After three cycles of chemotherapy, patient succumbed in view of consolidation and renal failure.

ÖzEkstramedüller plazmasitom nadir görülem plazma hücreli bir neoplazidir. Sıklıkla üst solunum yollarını tutar, göz tutulumu ise son derece nadirdir. Bu bildiride orbital kaynaklı bir ekstramedüller plazmasitom olgusu sunduk. 55 yaşında erkek hasta, sol gözde şişlik nedeniyle yapılan tetkiklerinde orbital yerleşimli 5×6 cm boyutlarında kitle tespit ediliyor. Kitlenin histopatolojik incelemesinde amiloid arasında bol miktarda plazma hücreleri izlenmiş olup kappa-lamda ile monoklonalite görülmüştür. Olgu plasma hücreli neoplazi olarak rapor edilmiş olup tetkiklerinde başka bir odak tesbit edilmediğinden soliter plasmasitom olarak kabul edilmiştir. Ekstramedüller plazmasitom orbital yerleşimli kitlelerde akılda tutulmalıdır. Prognozu farklılık gösterdiği için tanısında sistemik bir plazma hücre diskrazisinin varlığı mutlaka sorgulanmalıdır.

Orbital involvement of multiple myeloma (MM) is uncommon, with most of those reported cases occurring at the time of initial diagnosis of MM. We present a case of an extramedullary plasmacytoma involving only the right lateral rectus of a patient who had been in disease remission. The patient presented with new-onset diplopia and an abduction deficit of the right eye, with mild proptosis. In light of her past medical history of MM, an orbital MRI was obtained. The MRI demonstrated an isolated finding of eccentric enlargement of the right lateral rectus muscle limited to the muscle belly with sparing of the tendinous insertions, leading to diagnosis of plasmacytoma. Patching of the involved eye to alleviate the symptoms of diplopia was instituted. Chemotherapy was initiated, followed by orbital radiation and stem-cell transplantation for coexisting systemic disease. The orbital symptoms of proptosis and diplopia resolved within 1 month of treatment.
