Oral lactoferrin for the treatment of sepsis and necrotizing enterocolitis in neonates
Abstract: The safety and efficacy of different preparations and doses of lactoferrin needs to be established in neonates. Well designed adequately powered randomized multicenter trials are needed to address the efficacy and safety of lactoferrin in the treatment of neonatal sepsis and necrotizing enterocolitis. These trials should evaluate long-term neurodevelopmental and pulmonary outcomes in addition to short-term outcomes (e.g. mortality).

“…56 Lactoferrin concentration is very high in human colostrum. 57 Oral lactoferrin prophylaxis has been found to reduce the incidence of late-onset sepsis in infants weighing less than 1500 g. 58 Recent work describes its effect on neonatal myeloid cells in their conversion to myeloid-derived-suppressor-cells, thus blocking intestinal inflammation and experimental NEC. 59 While large-scale randomized clinical trials are needed, current evidence does not support a protective effect against NEC of exogenous lactoferrin when given alone.…”
