Optic Nerve Head Drusen and Visual Field Loss in Normotensive and Hypertensive Eyes

Abstract: VFL occurs more frequently in eyes with ONHD that also have OHT. Eyes with grade III ONHD are at increased risk for VFL compared to eyes with grade I drusen with the same intraocular pressure status. Patients with OHT and ONHD should undergo close surveillance for disease progression and be treated appropriately to prevent additional VFL.

“…Considering the prelaminar portion of the optic disc head to be relatively unexpandable, the presence of the calcified hyaline bodies will directly compress the surrounding vessels and axons (the crowded disc; Auw-Haedrich et al 1996;Spencer 1978). The extent of this compression has been thought to have clinical implications, as the more crowded the disc, the more prone these patients are to have a vascular-related complication and/or to further damage the neuron conductivity (Davis & Jay 2003;Grippo et al 2008). Our results seem to support this notion.…”
