Abstract: Purpose To evaluate the anatomical and visual outcomes in optic disc pit maculopathy following pars plana vitrectomy (PPV) with inverted internal limiting membrane (ILM) flap Methods Retrospective interventional case series of 10 patients diagnosed with serous macular detachment secondary to optic disc pit who underwent PPV with inverted ILM flap and were followed up for a year. Results A p-value of less than 0.05 was defined as statistically significant. … Show more

“…In this case series of 13 patients with ODP-M submitted to different surgical techniques, most patients had visual improvement within 3 years of surgery and these outcomes were sustained over time. We found that patients with signs of ODP-M chronicity (patients 4, 8, and 9) had worse postoperative BCVA, but also, patients with worse preoperative BCVA had better visual improvement, in accordance with other findings [ 4 , 5 , 6 ]. Despite some case series describing spontaneous resolution of the OPM [ 7 , 8 ], early surgical intervention can prevent prolonged damage to the photoreceptors [ 4 , 9 ].…”

“…The ILM flap and ILM stuffing techniques provide a physical barrier against the flow of fluid [ 5 ], and they can also act as a scaffold for the proliferation of Müller cells and consequential gliosis, further contributing to the barrier [ 10 ]. Previous comparison studies have shown that ILM stuffing of the pit had better or faster surgical outcomes than ILM peeling alone [ 18 , 19 , 20 ].…”

“…However, if complications arise that threaten the patients' vision, surgical treatment with pars plana vitrectomy associated with drainage of subretinal fluid and gas tamponade seem to be effective measures to control these cases. The removal of the internal limiting membrane can be justified by the anteroposterior traction it exerts on the retina, which facilitates the transmission of fluid from the optic pit to the macular region [15,17,19,20]. Surgical approach was not necessary in the patient in the present report, see the early diagnosis, the mild retinal involvement and the absence of ocular symptoms, with follow-up being carried out with annual consultations and imaging exams.…”

“…Surgical management is usually performed at the time of diagnosis if there are retinal involvements that justify such an approach. Most of the cases that present with good visual acuity and without significant serous detachment, only a conservative approach with regular consultations with imaging exams proves to be sufficient [14,18,19,20].…”

“…Sborgia et al 14 suggested that the inverted ILM-flap lying over the pit, probably because the small sized ILM-flap, could be not enough to cover the pit, so a large ILM-flap involving or sparing the foveal site could be used. [14][15][16] However, the occurrence of a macular hole was reported as possible complication of this approach especially in cases of ODPM with extremely thinned outer retina. 17,18 The first paper describing hAM use in ODPM was published by Rizzo and coworkers.…”
