Abstract: Background: The frozen elephant trunk (FET) technique was introduced in Hannover Medical School in 2001 to treat patients with complex aortic arch pathologies. Since 2012, we primarily use the trifurcated Thoraflex Hybrid FET graft. In this article, we report our experience with the trifurcated FET graft. Methods: Between November 2012 and September 2018, 211 patients underwent FET implantation with the trifurcated Vascutek Thoraflex Hybrid graft. The indications for surgery were: degenerative aneurysms in 68 p… Show more

“…2 The risk of causing spinal cord ischaemia (SCI), stroke, acute kidney injury, or in-hospital death are particularly pertinent -these are well-documented, common complications associated with arch repair. 3,4 It is therefore prudent that Mousavizadeh and colleagues have opted to carry out a meta-analysis and systematic review into this relationship.…”

“…Classical surgery favors orthotopic reconstruction in the majority of cases, and the current notion in the endovascular world is that approaches that mimic nature, with 2 or even 3 branches, will most likely be superior in the long run. 5 Two out of the authors' 9 patients experienced cerebrovascular events not only during the procedure, but also at the midterm follow-up. The reason for these periinterventional adverse events may be atherosclerotic changes, but it is disturbing that the same patients experienced repeat stroke in the midterm, after the aortic arch had been excluded from circulation.…”

“…Thoraflex™ Hybrid (Terumo Aortic) is a dedicated FET device that joins a distal endovascular nitinol ringed stentgraft with a proximal 4-branched surgical graft and has been used in acute and chronic settings to reduce circulatory arrest time and improve cerebral and systemic perfusion. 1 We have implanted over 70 Thoraflex Hybrid devices and been challenged by reimplantation of the supra-aortic trunk (SAT) vessels, especially the left carotid (LCA) and left subclavian (LSA) arteries, their chest positioning, and management of cerebral perfusion. To address this, we requested a custom-made configuration and describe here the first experience.…”

“…The frozen elephant trunk (FET) hybrid procedure has developed to treat both segments, sometimes with a definitive aortic reconstruction in one stage or in anticipation of subsequent completion with thoracic endovascular repair (TEVAR). Thoraflex™ Hybrid (Terumo Aortic) is a dedicated FET device that joins a distal endovascular nitinol ringed stent‐graft with a proximal 4‐branched surgical graft and has been used in acute and chronic settings to reduce circulatory arrest time and improve cerebral and systemic perfusion 1 . We have implanted over 70 Thoraflex Hybrid devices and been challenged by reimplantation of the supra‐aortic trunk (SAT) vessels, especially the left carotid (LCA) and left subclavian (LSA) arteries, their chest positioning, and management of cerebral perfusion.…”
