Opacification of Scleral-Sutured Akreos Ao60 Intraocular Lens After Vitrectomy With Gas Tamponade: Case Series

Abstract: Although still an excellent option for the management of surgical aphakia in the absence of capsular support, caution should be taken when using a hydrophilic secondary intraocular lens, such as Akreos AO60, during vitreoretinal surgery if gas is required.

“…Several reports described the opacification occurred after intravitreal gas injection during pars plana vitrectomy (PPV), the surgical procedure performed for treating diseases of the posterior segment of the eye [ 6 , 23 , 32 , 36 , 68 , 69 , 70 , 71 , 72 ]. The typical appearance of the opacification in most cases was similar to that occurring after DSAEK/DMEK [ 6 , 17 , 23 , 32 , 36 , 62 , 68 , 69 , 70 , 71 , 72 ]. Because the opacification appears mainly on the anterior surface of the IOL, although gas is injected into the vitreous, it is presumed that gas is likely to migrate into the anterior chamber through damaged zonular fibers [ 70 ].…”

“…Opacification of hydrophilic acrylic IOLs following intraocular gas injection has been described not only after intracameral injection of air or gas. “…In relation to PPV, either combined with phacoemulsification or alone, it is postulated that intracameral gas could contribute to the calcification of the surface of the IOL . Although the exact mechanism of calcification is under investigation, one hypothesis is that local damage to the hydrophilic IOL surface due to direct contact with air/gas at the exposed area may lead to Ca/P deposition from the aqueous humour .…”

“…Bausch & Lomb Akreos AO60 hydrophilic acrylic lens contains 4 eyelets allowing 4 point fixation. However, these lenses undergo calcification and opacify when in contact with intraocular gas or air [26]. Given that aphakic patients often have coincident retinal pathology and might be at increased risk for retinal detachment repair this might be an important consideration when deciding on the optimal lens and fixation technique.…”
