Background: Increase in the incidence of prior caesarean section and other obstetric interventions have contributed to a rise in the number of pregnancies complicated by placenta previa and its associated adverse maternal and perinatal outcome. This study compared the incidence of placenta previa, associated risk factors, placental position, complications and feto-maternal outcome in scarred and unscarred uterus. Objective of present study was to compare the antepartum, intrapartum and postpartum complications in placenta previa between previously scarred uterus and unscarred uterus.Methods: This was a prospective “nested” case control study for a period of 18 months, conducted at SAT Hospital, Medical College, Thiruvananthapuram with diagnosed cases of placenta previa with previously scarred uterus and without a previously scarred uterus. Statistical method used for analysis was chi-square test and students test wherever appropriate. P<0.05 was taken as significant.Results: In this study of 242 patients, there were 134 in the case group and 108 in the control group. The major types of previa were seen in the previously scarred uterus compared to unscarred uterus (55.9% versus 37.9%). Occurrence of recurrent APH (58.9% Vs 41.6%), postpartum haemorrhage (69.4% Vs 48.1%), adherent placenta (5.9% versus 0%) and need for additional operative procedures (66.4% versus 53.7%) including obstetric hysterectomy, need for blood transfusions, postoperative complications (9.7% versus 1.8%), long postoperative hospital stay were all significantly associated with cases of placenta previa in previously scarred uterus.Conclusions: The primary caesarean rate needs to be reduced in order to reduce future pregnancy problems like adherent placenta and caesarean hysterectomy. Such patients need early referral and management in a tertiary health centre with all facilities available for a better maternal and neonatal outcome.

INTRODUCTIONPlacenta praevia is characterized by the abnormal placenta overlying the endocervical os, and it is known as one of the most feared adverse maternal and fetalneonatal complications in obstetrics.1 Women with placenta praevia are at an increased risk of second trimester vaginal bleeding.2 Placenta praevia is a source of drastic pregnancy, fetal and neonatal complications, e.g. peripartum hysterectomy, blood transfusion, postpartum hemorrhage, and placenta accreta for the mother, prematurity and intrauterine growth restriction for the fetus and increase in neonatal mortality rates in singleton pregnancies. 3Pathophysiology of placenta praevia is uncertain.Moreover, there appears to be an association between endometrial damage and uterine scarring and subsequent placenta praevia. We have conducted this study to find out the incidence of placenta praevia in Najran University Hospital. 4 METHODSThis study is a cross-sectional descriptive, one-year retrospective and one-year prospective study, carried out from April 2014 to April 2016. All pregnant women attended the antenatal care clinic in Najran University Hospital during the study period, were included. The data was collected in a designed questionnaire after informed consent from all patients in the prospective period.The patient's records in the retrospective period were the source of data. We carried out this study to find the impact in our area and the possible risk factors for this serious obstetric problem. Methods: This study is a cross-sectional descriptive, one-year retrospective one-year prospective study, carried out from April 2014 to April 2016. All pregnant women attended the antenatal care clinic in Najran University Hospital during the study period, were included. Results: The incidence of placenta praevia was found to be 2.7%. AMONG the study group, the most affected age group was between 20-29 years. Previous placenta praevia and previous caesarean section were the significant risk factors P=0.00 and 0,017 repectively. Conclusions:The incidence of placenta praevia in Najran KSA in this study was high, compared to other regions in Saudi Arabia and the majority of the international figures. Caesarean section and previous placenta praevia were the significant risk factors for placenta praevia in this study.

Cesarean section is one of the most common obstetric procedure in worldwide. In addition, the increasing increased rate of cesarean section has been observed in recent studies. Maternal and fetal mortalitiy and morbidity associated with cesarean section is an important health problem in all over the world. The primary obstetric indication for cesarean section is previous cesarean section. Vaginal birth after cesarean section can be alternative delivery method for women who have previous cesarean section. But, the risks of this type delivery must be carefully evaluated. As a result, increasing rate of cesarean section can be decreased with vaginal birth after cesarean section. Key words: Vaginal birh after cesarean section, indication, contraindication, complication, repeat cesarean section ÖZET Sezaryen dünyada en yaygın obstetrik uygulamalardan biridir. İlave olarak son çalışmalarda sezaryen oranında artış gözlenmektedir. Maternal ve fetal mortalite ve morbidite ilişkili olan sezaryen doğum tüm dünyada önemli bir halk sağlığı problemidir. Sezaryen doğum için başlıca endikayon geçirilmiş sezaryen doğumdur. Sezaryen sonrası vaginal doğum geçirilmiş sezaryen operasyonu olan hastalarda alternatif bir doğum metodu olabilir. Fakat doğumun bu tipinin riskleri dikkatlice değerlendirilmelidir. Sonuç olarak, artmış sezaryen doğum oranı sezaryen sonrası vaginal doğum ile azaltılabilir.
