Normal live birth after vitrified/warmed oocytes intracytoplasmic sperm injection with immotile spermatozoa in a patient with Kartagener's syndrome

Abstract: The present article is a report on two cases of male Kartagener's syndrome enrolled in intraconjugal IVF programme due to akinetospermia. Viable spermatozoa were selected using a hypo-osmotic swelling test (HOST) and pentoxifylline activation and subsequently microinjected into vitrified/warmed oocytes. The treatment enabled one of these two couples to achieve a pregnancy and to give birth to a healthy baby girl.

“…Therefore, effective preservation of the fertility of viable but immotile spermatozoa is a major challenge facing IVF. As is known, completely immotile spermatozoa are often found in IVF treatment cycles involving ejaculate samples in patients with conditions such as primary cilia dyskinesia [91011], which is a rare autosomal recessive multisystem disorder characterized by ultrastructural and functional defects of the ciliated cells of the body. Completely immotile spermatozoa also occasionally occur in surgically extracted testicular spermatozoa [12], which can be attributed to very heterogeneous events and are a consequence of oxidative stress, infection, or numerous other factors [13].…”

“…In patients suffering from PCD or Kartagener syndrome, however, male infertility derives from ultrastructural defects of the flagella, e.g., absence of the inner dynein arms. This scenario automatically leads to severe asthenozoospermia [4][5][6] or much rather to complete absence of motile gametes [7][8][9][10][11][12][13][14].…”

“…first evaluation of oocyte vitrification in France [9], it has been used in routine management of infertility and proposed to couples in cases where a large number of mature oocyte was collected, embryo transfer was contraindicated for medical reasons and when no or to few spermatozoa were available for intracytoplasmic sperm injection (ICSI) on the day of oocyte retrieval [9,10].…”
