Neurovascular decompression for idiopathic tarsal tunnel syndrome: technical note

Abstract: Objective-The surgical outcome of idiopathic tarsal tunnel syndrome (TTS) is reported to be worse than that attributable to ganglion, tarsal coalition, or tumour, and therefore further development in the surgical treatment for idiopathic TTS is considered to be necessary. Here the eYcacy of neurovascular decompression for patients with idiopathic TTS is evaluated. Methods-Twelve feet from nine patients with idiopathic TTS were treated. The patients were aged 52-78 years (mean 64.6 years), and all of them compl… Show more

“…The posterior tibial neurovascular complexes are combined tightly in the tarsal tunnel because they are often attached to the fibrous septa with in the tunnel [14,15]. In the present case, possibly the medial plantar portion of tibial nerve was penetrated by a branch of posterior tibial artery.…”

“…Kohno e cols. 13 , em 2000, sugeriram a existência da compressão neurovascular na síndrome do túnel do tarso idiopática e, devido a isso, reportaram uma técnica de interposição de fragmento de tecido gorduroso entre os nervos e as veias. Na presente série, considerando esta possibilidade, o autor preferiu a coagulação e secção das veias que cruzavam o trajeto nervoso.…”

“…Invasive procedures can include injections at the tarsal tunnel and sympathetic ganglion blocks. As a last resort, surgical release of the tunnel retinaculum can be implemented, 237 where the technique can be improved when using an insertion of a fat graft between the vessels and the tibial nerve 238 …”
