A localized form of ‘Pagetoid reticulosis’ (Woringer-Kolopp’s disease) is described morphologically. It can be shown that ultrastructurally the ‘Pagetoid cells’ are heterogenic and that different cell types can be distinguished: there are lymphoid cells, which often look like Lutzner cells, and nonlymphoid cells which can be characterized as indeterminate cells or Langerhans’ cells. Often a close topographical relation between lymphoid and nonlymphoid cells can be demonstrated. This finding is suggestive for a functional interaction between the lymphoid cells, which are most probably T cells, and indeterminate cells and Langerhans’ cells, which are known to be important for the thymus-dependent immune reaction. The presence of the nonlymphoid cell population in the epidermis in Woringer-Kolopp’s disease seems to build up the specific microenvironment for lymphoid cell reaction.

A typical clinical and histologic case of so-called ‘pagetoid reticulosis’ or Woringer-Kolopp disease is reported. This is a chronic cutaneous condition usually with a single focus or at most with a few patches confined to a circumscribed region. A disseminated form has been described as Dupont-Vandaele type, but its relationship to the classic Woringer-Kolopp disease is uncertain. The nosological location of pagetoid reticulosis has not been established, but there are reasons to regard it as a particular and well-defined entity. In relation to the nature of this condition and to the origin of the ‘pagetoid cells’, it is tempting to consider the possibility that Woringer-Kolopp disease might be a lymphoproliferative epidermotropic disorder, classifiable as a benign form of ‘cutaneous T-cell lymphoma’, but the early histologic changes are rather indicative of an epidermal origin.

A case of pagetoid reticulosis is presented. Histopathology showed infiltration of the epidermis by mononuclear cells. Twenty percent of the mononuclear cells showed the presence of lysozyme indicating a histiocytic origin. Electron microscopy confirmed the presence of lymphocytes and histiocytes but these cells were outnumbered by Sézary cells. The presence of large numbers of Sézary cells indicates that pagetoid reticulosis is a cutaneous T-cell lymphoma closely related to mycosis fungoides.
