Multiple cutaneous plasmacytomas following an autologous peripheral stem cell transplant

Abstract: We describe the unusual development of multiple cutaneous plasmacytomas following treatment of IgA lambda myeloma with myeloablative therapy and a peripheral blood stem cell autograft. Cutaneous metastatic spread was evident despite bone marrow remission. Treatment with an autograft may have contributed to the cutaneous relapse.

“…Four out of seven patients in these cases died from rapid progression of the disease. Concurrent chemoradiation therapy is considered a treatment of choice in patients with extensive involvement7101213. We herein report an interesting case of cutaneous plasmacytoma at the surgical site of fracture repair.…”

“…Metastasis of tumor at surgical sites can occur by different mechanisms. It may be due to etiologic factors of local surgical site such as direct wound contamination and manipulation of tumor cells and systemic condition of the patient such as alteration in immune response713. A theory has been proposed to explain the immunologic process of plasmacytoma metastasis at wound sites after chemotherapy regimen.…”

“…Hence, the newer drugs are unable to overcome the impact of this aspect of disease biology and ultimately the condition becomes resistant to salvage chemotherapy [10]. The pathogenesis of eMM post-ASCT is uncertain but may involve re-seeding of myeloma cells (by infusion of stem cells contaminated with MM cells during harvesting) and metastasis of residual myeloma due to incomplete myeloablation [14]. …”

“…The transformation from primarily medullary to extramedullary myeloma involvement after autologous or allogenic SCT is common, suggesting selection of plasma cell with atypical homing, or localized disease not eradicated by the conditioning (6, 7). Localized relapse can happen even in absence of systemic progression (8–10). Risk factors to develop extramedullary plasmacytomas are younger age, male gender, Bence‐Jones myeloma, lambda light chain, advanced stage, extensive skeletal involvement, and non‐secretory myeloma (2, 3).…”
