MR Lymphangiography: A Practical Guide to Perform It and a Brief Review of the Literature from a Technical Point of View

Abstract: We propose a practical approach for performing high-resolution MR lymphangiography (MRL). We shall discuss and illustrate the technical approach for the visualization of lymphatic vessels in patients suffering from lymphedema, how to distinguish lymphatic vessels from veins, and MRL role in supermicrosurgery treatment planning. A brief review of literature, from a technical point of view, is also reported.

“…Recently, magnetic resonance lymphangiography (MRL) has been added to the armamentarium to help localise the lymphatic channels and venules for LVA [11] . A mixture of paramagnetic contrast medium containing gadobenate dimeglumine and lignocaine is injected subcutaneously/intradermally in the webspaces of both feet and MRI is performed.…”

“…Die MRT-Lymphografie ist zur 3-dimensionalen Darstellung der Lymphgefäße und möglicher Lymphabflussstörungen geeignet. Eine MRT-Untersuchung ohne Kontrastmittel wird mit einer T2-gewichteten Sequenz mit einer speziellen MIP-Projektion ("Maximum intensity projection") gefahren, um einen Austritt der Lymphflüssigkeit in das umliegende Gewebe darzustellen [9] (▶ Abb. 4).…”

“…The main drawback of this technique is the low image resolution and the procedure accompanied of ionizing radiation for patient. More recently, magnetic resonance lymphography (MRL) has been used to effectively examine the lymph vessels and identify their morphology[11] [12]. Different imaging techniques are used to diagnose lymphatic disorders (lymphangiography, lymphoscintigraphy, computed tomography (CT), MRL, positron emission tomography (PET)/CT and ultrasound imaging).…”
